Abstract

V magistrskem delu so v teoretičnem delu predstavljeni etika, etična načela, etične dileme, ki se pojavljajo pri delu na področju psihiatrične zdravstvene nege, opredeljeni so posebni varovalni ukrepi (PVU) ter zakonodaja, ki ureja uporabo PVU na področju psihiatrične zdravstvene nege. V empiričnem delu smo želeli ugotoviti, kako pogosto se pri zdravstveni negi na psihiatričnem področju pojavljajo etične dileme. Prav tako je bil namen raziskave ugotoviti, ali izvedba PVU predstavlja težavo oz. skrb pri zdravstvenemu osebju. Ali medicinske sestre menijo, da gre pri uporabi PVU za nespoštovanje človekovega življenja, kršitev načela dostojanstva in avtonomije pri pacientu in osnovnih človekovih pravic, ter če so zdravstveni delavci, ki izvajajo in sodelujejo pri izvajanju PVU, seznanjeni s smernicami za izvedbo posebnih varovalnih ukrepov. Metodologija raziskovanja. Uporabljena je bila kvantitativna metodologija raziskovanja. Pri tem smo uporabili vprašalnik, sestavljen iz osemnajstih vprašanj pol odprtega tipa. V raziskavo je bilo vključenih 25 članov tima zdravstvene nege na varovani enoti v Domu Lukavci. Rezultate ankete smo obdelali s pomočjo računalniških programov Microsoft Office Word 2010, Microsoft Office Excel 2010 ter IBM SPSS Statistics 19. Rezultati raziskave. Rezultati raziskave so pokazali, da je večina vprašanih (91,8 %) mnenja, da so težke odločitve, ki jih sprejemajo na svojem delovnem mestu predvsem odločanje med načeli: pravičnosti, zasebnosti in zaupanja pacientov in dejanskimi možnostmi. Zaposleni na varovani enoti Doma Lukavci so v večji meri (56 %) mnenja, da jim izvedba PVU redko predstavlja težavo oz. skrb, (72 %) vprašanih meni, da pri uporabi PVU gre tudi za kršitev načela dostojanstva in avtonomije pri pacientu. Sklep. Ljudje se med seboj razlikujemo v marsičem, vsem pa je skupno človeško dostojanstvo, do katerega imamo vsi enako pravico. V odnosu do bolnih, trpečih in ranljivih je spoštovanje človeškega dostojanstva še posebej pomembno. Medicinske sestre se pri svojem delu vsakodnevno srečujejo s situacijami, ki so povezane z zdravstvenim stanjem pacienta. Za profesionalno opravljanje svojega poklica morajo razviti visoko stopnjo etičnega zavedanja. Pri presojanju etičnih odločitev jim je v pomoč profesionalni kodeks etike, ki mora biti vodilo pri presoji njihovega delovanja.

Secondary abstract: Theoretical part of the following master's thesis consists of ethics, ethical principles and ethical dilemmas that occur during the work in the field of psychiatric health care. Special precautionary measures (SPM) and legislation that regulates the use of SPM in the field of psychiatric health care are also defined. Empirical part is based on our establishment of the frequency of ethical dilemmas in the field of health in psychiatric care . The purpose of this research was also to establish whether the SPM present difficulties or concern with the medical personnel. Whether the nurses believe that the application of SPM leads to violation of human life, principle of dignity and patients’ autonomy as well as violation of basic human rights and whether the medical personnel, who perform and take part in the SPM performance, are familiar with the guidelines for the performance of special precautionary measures Methodology of the research. Quantitative methodology has been used. In the process we have used a questionnaire with eighteen open-ended questions. 25 members of medical personnel from the secure wards of Dom Lukavci were included in the survey. The results were processed with the help of computer programmes Microsoft Office Word 2010, Microsoft Office Excel 2010 and IBM SPSS Statistics 19. Results of the research. The results have shown that the majority (91.8 %) share the same opinion i.e. the difficult decisions they make in their working place are mostly decisions between different principles: fairness, privacy and patients’ trust and actual possibilities. Employees of the secure wards of Dom Lukavci think (56 %) that the execution of SPM rarely represents any troubles or concern, (72 %) of the participants feel that when using SPM they violate patients’ principles of dignity and autonomy. Conclusion. People are different in many things, nevertheless the human dignity is a thing we all have in common and are all entitled to. Respect of the human dignity becomes very important in a relation to the sick, stricken and vulnerable. Nurses encounter situations connected to the patient’s state of health on a daily basis. To work in a professional manner they have to develop a high level of ethical consciousness. To estimate their ethical decisions they relate to the professional code of ethics which has to guide them in the judgement of their own actions.
