Abstract
Teoretična izhodišča: Bistvo paliativne oskrbe je izvajanje sistematične celostne pomoči kronično ali neozdravljivo bolnim in umirajočim pacientom s ciljem boljše kakovosti življenja pacienta in njegovih bližnjih. Namen magistrskega dela je predstaviti vlogo patronažne službe pri paliativni oskrbi in z raziskavo ugotoviti, ali se paliativna oskrba izvaja skladno s sprejetim Državnim programom paliativne oskrbe, ter podati predloge za njen nadaljnji razvoj iz vidika organizacije in koordinacije.
Metodologija raziskovanja: Uporabili smo kvantitativno metodologijo. Kot instrument raziskovanja smo uporabili anketni vprašalnik, sestavljen je bil iz vprašanj odprtega in zaprtega tipa. Raziskovalni vzorec je zajemal 58 zaposlenih v Organizacijski enoti Patronažno varstvo enega izmed večjih zdravstvenih domov v Sloveniji. Pridobljene podatke smo analizirali s pomočjo programov MS Excel 2013 in IBM SPSS Statistics 20.0.
Rezultati: Rezultati raziskave so pokazali, da se paliativna oskrba še ne izvaja skladno s smernicami in standardi zastavljenimi v Državnem programu paliativne oskrbe. Ugotovljeno je bilo, da predvidena enotna dokumentacija celostne obravnave paliativnih pacientov in tim specialistične paliativne oskrbe v zdravstvenem domu, kjer smo opravljali raziskavo, ne obstajata. Dodatno je bilo ugotovljeno, da se patronažne medicinske sestre pri svojem delu srečujejo s problemi glede koordiniranja paliativne oskrbe in da so se pripravljene dodatno izobraževati glede organizacijskih in komunikacijskih veščin, katere potrebujejo za kakovostno izvajanje in koordiniranje paliativne oskrbe.
Sklep: Državni program paliativne oskrbe je primerljiv z nacionalnimi načrti drugih držav Evropske unije in je dobro zastavljen, žal pa v Sloveniji ni v celoti prenesen v prakso. Pri obravnavi paliativnih pacientov pogosto prihaja do situacij, ko patronažne medicinske sestre iščejo edinstvene rešitve za kakovostno obravnavo paliativnih pacientov. Za celovito obravnavo paliativnih pacientov bi bilo smiselno oblikovati enoten model na nivoju države, ki bi moral vključevati uporabne, poenotene standarde in klinične poti.

Proposal of a Palliative Care model in Community Health Nursing |

Theoretical basis: Palliative care basis is in execution of holistic help to chronic or terminally ill or dying patients with the goal of improving patient’s life quality and life quality of those close to patient. The purposes of the master thesis are the introduction of the community health nursing in palliative care and with a research determined conclusion about execution of the palliative care in accordance with the National Programme for Palliative Care and propositions for its further development from the organizational and coordinating standpoint.
Research methodology: A quantitative methodology was used. A research method was based on a survey questionnaire that contained open and closed question types. Research sample included 58 employees in organizational unit Patronažno varstvo (Community Health Nursing) in one of the bigger community health centers in Slovenia. Information obtained was analyzed with the Ms Excel 2013 and IBM SPSS STATISTICS 20.0. programs.
Results: Research results have shown that the palliative care is not yet implemented in accordance to guidelines and standards in National Programme for Palliative Care. It was determined that the foreseen unified documentation of holistic care for the palliative patients and the team of specialists in palliative care are non-existent in the health centre where the research took place. Furthermore it was established that the community health nursing nurses face many problems in coordination of the palliative care and are prepared to further educate in organizational and communicational skills which they need for quality implementation and coordination in palliative care.
Conclusion: National Programme for Palliative Care is comparable to national plans in other EU countries and is well pledged. Unfortunately it is not wholly transferred to praxis. In the treatment of palliative patients often comes to situations in which community nursing nurses seek unique solutions for quality palliative patient treatment. For comprehensive palliative patients care establishment a unified model at the national level would be reasonable and should include useful, unified standards and clinical paths. |
