Abstract

Patronažna medicinska sestra in socialna delavka na terenu pogosto sodelujeta. njuno medsebojno delovanje ima velik pomen za starostnika. Patronažna medicinska sestra pri skupni obravnavi pokriva zdravstveni del, socialna delavka pa socialni vidik. Vsaka od njiju nato predlaga rešitve na koncu pa se individualno odločita, katere rešitve so najprimernejše za določenega starostnika. Pri celotni zdravstveno-socialni obravnavi sodelujejo tudi svojci. Najpogostejši rešitvi sta urejanje domskega varstva in ureditev pomoči na domu.

Secondary abstract: Community nurses frequently cooperate with social workers and that their cooperation is of great importance to the eldery. . The community nurse tends to the medical aspect of care, whereas the social worker covers the related social issues. Each of them proposes appropriate solutions, which are carefully considered and decided upon on the basis of each elderly person. The overall medical and social treatment also includes the person’s relatives. We have discovered that the most common solutions for socially disadvantaged elderly people are represented by home care and institutional care.
