magistrsko delo Management v zdravstvu in socialnem varstvu
Abstract
Namen magistrskega dela je najti odgovor na mnoga vprašanja, s katerimi se zdravstveno osebje sooča, predvsem pa, kdaj je ogrožena varnost skupnosti, ko etična načela zavezujejo medicinsko sestro, da krši poklicno molčečnost. Kako najti rešitev med kolizijo etičnih pravil in pravno obvezujočih aktov? Kdo ščiti medicinsko sestro, ko se pogosto ravna zgolj etično in brez zakonske podlage varnosti? Iz literature je razvidno, da je rezultat tehtanja interesov, ki so si v koliziji, odvisen od osebnosti medicinske sestre in njenega lastnega vrednostnega sistema. Razlog za to je zagotovo trditev, da so si varovanje osebnih podatkov in pravice pacientov v koliziji z dolžnostjo varovanja drugih oseb.
Ko govorimo o varovanju drugih oseb, lahko prištevamo v to skupino ne le paciente, ampak tudi ostale zaposlene v zdravstveni negi oz. vse, ki so neposredni aktivni dejavni člen v tem procesu. Medicinska sestra sodeluje s celotnim timom, ki je vključen v proces zdravljenja pri bolniku.
Vsaka zdravstvena ustanova ima svoj informacijski sistem in svojo bazo podatkov. Informacije v tem primeru ne »potujejo« skupaj s pacientom po zdravstvenih ustanovah, kjer se izpeljuje diagnostika. Zdravstvena kartica je osnovna baza podatkov, katere zapis je skromen. Vsak zdravstveni delavec ima vpogled v zdravstveno kartico s svojo profesionalno kartico, ki ima omejitev dostopa vpogleda v pacientove podatke. Možnost okužbe nalezljivih obolenj in prenosa le-teh bi mogli uvrščati med osnovne podatke, tako kot je osebno zavarovanje pacienta. Ta podatek bi moral biti dostopen in na vpogled zdravstvenemu osebju.
Rešitev celotnega problema je delno torej v enotnem informacijskem sistemu celotnega zdravstvenega sistema ali vsaj v omogočenem prenosu podatkov med različnimi informacijskimi sistemi.

The purpose of this Master’s degree is to finde answers on a lot of different questions. Specialy on a questions, which are the medical stuff deeling with: When is the safety of the community at risk? When ethical principles bound nurses to violation professional secrecy?
How to finde the solution between the opposite between ethical principles and legal acts? Who protects nurses, when they act mostly ethical and they don’t have legal support?
We can see from the literature, that the resault of consideration between different interests, depends from personality of the nurse and her moral system.
The main reason to all that, is the claim, that protection of personal data and patient rights are the opposite with duty to protect others.
When we talk about ways how to protect other people, we are not talking just about other patients, but we are thinking about all the medical and none medical staff which are working around patient on departments.
Every medical center have their own information system and data base.
The informations about patients are not travelling with them. The only thing which is always with patient is medical card, wich is the mainly base of patient data, but unfortunately, it is to modest. Every doctor and some nurses, have professional card, and they can see data written on medical card. Of course, defferent profile of workors, have different competence, what they are aloud to see. Contagious deseases should be included on medical card like alergys and other emergency data.
The big solution is in one big information system. We are talking about one system for all medical centers in Slovenia. |
