Abstract

V diplomskem delu so predstavljeni paliativna zdravstvena nega kot integralni del paliativne oskrbe bolnikov z napredovalo kronično neozdravljivo boleznijo, vključevanje paliativne obravnave v domove starejših občanov in vloga svojcev pri izvajanju paliativne oskrbe. V empiričnem delu smo želeli ugotoviti, ali se člani negovalnega tima čutijo usposobljeni za delo z bolnikom z napredovalo kronično neozdravljivo boleznijo ter ali čutijo potrebo po vključitvi paliativne oskrbe v dom upokojencev. V raziskavi smo uporabili kvantitativno metodologijo in metodo anketiranja v obliki vprašalnika. Anketni vprašalnik je vseboval 15 vprašanj zaprtega, odprtega in polodprtega tipa. V raziskavo je bilo vključenih 45 anketirancev, potekala je v mesecu avgustu 2016 v enem izmed domov upokojencev. V vzorec se je uvrstilo 36 pravilno izpolnjenih anketnih vprašalnikov, kar predstavlja 80 % realizacijo. Rezultati so predstavljeni pisno in v obliki tabel s pomočjo računalniškega programa Microsoft Excel, kjer smo kot statistično metodo uporabljali frekvenčno analizo. Raziskava je pokazala, da so potrebe po paliativni oskrbi prepoznane, žal pa je prepoznana tudi problematika. Pomanjkanje specifičnega znanja, izobraževanj ter organizirane povezanosti na vseh nivojih zdravstva ne morejo zadostiti vse večjim potrebam stanovalcev z napredovalo kronično neozdravljivo boleznijo. Dodaten problem predstavlja kadrovska podhranjenost zdravstveno negovalnega osebja. Ob odsotnosti zdravnika je zdravstveno negovalno osebje prepuščeno samim sebi ter svojcem, ki gojijo različna pričakovanja in poslabšanja zdravstvenega stanja svojih ljubljenih sprejemajo zelo različno. Posledično pa se tako dodatno bremeni dežurne zdravniške službe, le – te pa ob napotitvah naprej, bremenijo bolnišnice. Nujno in neizogibno bo potrebno izboljšati obveščenost oziroma seznanjenost stanovalca in svojcev z napredovalo boleznijo, prognozo in pričakovanji. Sprejemanje slabe diagnoze in še slabše prognoze je prav gotovo izredno težka situacija, vendar bi pravočasni družinski sestanki tako sprejemanje prav gotovo olajšali. Pričakovanja bi bila realnejša, trenutki stanovalcev z razumevajočimi svojci pa neprecenljivi.

Secondary title: INTRODUCING PALLIATIVE CARE IN THE NURSING HOME
Secondary abstract: Palliative health care as the integral part of the palliative care of patients with any advanced chronic incurable illness, the introduction of the palliative care into nursing homes and the role of the family in the palliative care is presented in this thesis. The empirical part of the thesis focuses on the question whether the members of the nursing team feel qualified enough to be working with people suffering from the advanced chronic incurable illness and if they feel the need to include the palliative care into the nursing home. We have used the quantitative research methodology and the survey methodology in the form of a questionnaire in the survey. The questionnaire consisted of 15 closed-ended, open-ended and semi-open questions. 45 surveys were subjected to this survey and it was carried out in August 2016 in one of the nursing homes. 36 questionnaires were correctly filled in and analysed, which represents 80%. The results are presented in a written form and in Microsoft Excel tables where we used the frequency analysis as a statistical method. The survey has shown there is a need for palliative care, however, it has also highlighted the problems associated with it. Firstly, there is a lack of specific knowledge, education and organization in all levels of health care which cannot satisfy the needs of growing numbers of residents with the advanced chronic incurable illness. Additionally, there is a serious problem of staff deficiency in the health care. In the absence of doctors, the health care staff and the family, who have different expectations and accept the deterioration of their loved ones' health differently, are left to themselves. Therefore, emergency health care and furthermore the hospitals are being burdened. It is essential and inevitable to inform the resident and his/her family about the advance of the illness, the prognosis and expectations. The acceptance of a bad and even worse prognosis must be very difficult but it could be made easier by having family meetings. If they were held at the right time, the residents' expectations would be realistic and the moments spent with their family invaluable.
