diplomsko delo visokošolskega strokovnega študijskega programa I. stopnje
Abstract
Okužbe sečil z enterobakterijami so pogoste, za usmerjeno zdravljenje je pomemben kratek čas do antibiograma.
Namen diplomskega dela je bil ugotoviti, ali je mogoče standardnemu enakovreden antibiogram enterobakterij izdelati v enem dnevu od sprejetja vzorca.
Metode. Vzporedno s standardnim postopkom, ki traja 2 dni (en dan za kultivacijo urina, en dan za standardni antibiogram (SAT), smo vzorce urinov cepili v viale z bujonom (NBJ) in jih inkubirali v analizatorju HB&L (Alifax): če smo zaznali rast gostote McFarland 0,5, smo naredili razmaz in če so bile vidne samo po Gramu negativne bakterije (NB), smo izdelali hitri antibiogram (HAT). Razlika: pri HAT smo inokulum gostote McFarland 0,5 pripravili iz NBJ, pri SAT iz kolonij urinokulture. Primerjali smo rezultate HAT in SAT.
Pripravili smo tudi vankomicinski reagent. Pri delu vzorcev smo preučili 4 možne strategije uporabe vial z vankomicinom ali brez, z ali brez razmaza po Gramu.
Rezultati. Cepili smo 230 urinov, pri 52 vzorcih smo v standardnem in hitrem postopku osamili eno vrsto enterobakterij – pri teh smo primerjali rezultate HAT in SAT. Skupno smo testirali 1030 kombinacij izolat/antibiotik in ugotovili 99,7 % skladnost. Velikih neskladnosti in zelo velikih neskladnosti ni bilo, ugotovili smo 0,3 % majhnih neskladnosti. Kriteriji za uspešno validacijo metode so izpolnjeni: več kot 90 % skladnosti in manj kot 3 % velikih ali zelo velikih neskladnosti.
Poleg NBJ smo 150 vzorcev cepili tudi v vankomicinski bujon - VBJ. Najbolj učinkovita strategija je nacepitev urina v VBJ, razmaz po Gramu in izdelava HAT, če so v preparatu vidne le NB.

Enterobacteriaceae are common cause of urinary tract infections. Short time to results of antimicrobial susceptibility testing (AST) is important.
Purpose of the thesis was to find out whether it is possible to do reliable AST of Enterobacteriaceae from urine sample in one day.
Method. In parallel with the standard procedure, which lasts 2 days (one day for cultivating urine and one day for standard AST (SAT), we inoculated samples in vials with broth (NBJ) and incubate them in HB&L Analyzer (Alifax). If we detected growth with density at least McFarland 0.5, Gram stain was used and if only Gram negative bacteria (NB) were visible, we made fast AST (HAT). The difference: in HAT, inoculum was prepared from NBJ, in SAT from colonies of urine culture. We compared the results of HAT and SAT.
We prepared vancomycin reagent and examined use of 4 possible strategies: use of vials with or without vancomycin, with or without a Gram stain.
Results. We inoculated 230 samples and in 52 only one species of Enterobacteriaceae was
isolated with both standard and expedited procedure. In these samples, results of HAT and SAT were compared: a total of 1030 antibiotic/isolate combinations were tested with 99.7% agreement and 0.3% of minor discrepancies were found. No major or very major discrepancies were found. Criteria for successful validation of the methods were fulfilled: more than 90% agreement and less than 3% of major or very major discrepancies.
In addition, we inoculated 150 samples in vancomycin broth - VBJ. The most effective strategy is use of VBJ, Gram stain of suspension and use of HAT, if only NB were visible in Gram stain. |
