Abstract

V magistrski nalogi je predstavljeno delovanje patronažnega varstva v Sloveniji s poudarkom na obremenjenosti kadra. V teoretičnem delu je najprej opredeljeno delovanje patronažnega varstva znotraj sistema primarnega zdravstvenega varstva, v nadaljevanju pa smo znotraj delovanja patronažnega varstva izpostavili pravno podlago za delovanje, kadrovske in delovne normative, financiranje ter preventivno in kurativno dejavnost v patronažnem varstvu. Raziskovalni del magistrske naloge temelji na kvantitativni metodologiji. Analizirali smo delovno obremenitev kadra, zaposlenega v patronažnem varstvu v Sloveniji, po podatkih o kadrovski zasedenosti in realizaciji delovnega programa med letoma 2005 in 2014. V analizi smo preverjali razlike v delovni obremenitvi kadra po posameznih regijah v Sloveniji in ugotavljali dejavnike, ki vplivajo na delovno obremenitev, in kadrovske normative, ki jih priznava plačnik. Ugotavljali smo tudi trenutne razlike v kadrovski zasedenosti v patronažnem varstvu v Sloveniji ter analizirali dejanski primanjkljaj diplomiranih medicinskih sester in zdravstvenih tehnikov glede na strokovne normative. Ugotovili smo, da je kader, zaposlen v patronažnem varstvu, v vseh regijah preobremenjen, saj je bil v desetih letih delovni program konstantno presežen povprečno za več kot 12,5 %. Ugotovili smo velike razlike v delovnih obremenitvah med posameznimi regijami. Po kadrovski zasedenosti glede na priporočila stroke pri izvajanju patronažne zdravstvene nege na terenu primanjkuje 394 izvajalcev v patronažnem zdravstvenem varstvu. Po opravljenih analizah predlagamo, da se pristopi k reševanju kadrovske problematike v patronažnem varstvu na podlagi analize obstoječega stanja kadra in glede na preseganje delovnega programa. Predlagamo, da se najprej razbremeni obstoječi kader, tako da se v neposredno prakso na terenu zaposli kader, ki ga plačnik finančno trenutno že zagotavlja, a iz različnih razlogov na terenu ne izvaja patronažnih obravnav. Predlagamo tudi, da se na ravni države čim prej izdela javna mreža delovanja patronažnega zdravstvenega varstva, ki bo predstavljala zemljepisno porazdelitev človeških in materialnih zmogljivosti ter s katero bo ljudem zagotovljena optimalna dostopnost patronažne obravnave. Predlagamo, da so poleg števila prebivalcev na posameznem območju merila pri postavljanju mreže v patronažni dejavnosti tudi demografska in socialna struktura prebivalstva, stopnja urbanizacije območja ter gospodarske možnosti. V magistrskem delu je predstavljen še predlog upoštevanja kriterija ruralnosti v pripravi javne mreže za patronažno dejavnost in pripravljen izračun potrebnega števila izvajalcev za eno od regij.

Secondary title: Workload of personnel in community nursing care
Secondary abstract: The present master's thesis deals with the community health nursing practice in Slovenia with the emphasis on the staff's workload. The theoretical part, which discusses the community health nursing practice within the primary health care system, is followed by a description of the legal basis for community nursing, of the staffing and working standards, of the financing of the service, as well as of prevention and treatment activities performed by community health nurses. The research part of the thesis is based on a quantitative methodology. An analysis was made of the workload of community health nurses in Slovenia, whereby the data on the number of employees and the implementation of work programme between 2005 and 2014 was taken into consideration. A comparison was made of the workload of employees in different regions in Slovenia. We established the criteria that affect the workload, and staffing standards acknowledged by the provider of funds. The differences in the number of employees in the community health nursing in Slovenia were determined, and the actual shortage of graduate nurses and medical technicians was analysed with regard to professional standards. It was discovered that the employees in the community health nursing are overworked in all Slovenian regions. During the period of ten years, the work programme had namely been exceeded by more than 12.5 % on average. We found out that there were significant regional differences in the workload of community health nurses. Considering the recommendations of the community nursing experts regarding the number of employees in the field, it was established that there is a total shortage of 394 providers of community health nursing care. Following the performed analyses, we proposed that the staffing shortage in the community health nursing should be dealt with on the basis of the analysis of the current situation as regards the number of employees and the exceeded work programme. Current employees should be relieved of the workload by employing new community health nurses in the field. The funds for the employment have already been granted, while the services are not being provided for various reasons. We also proposed that a public community nursing network be established at the state level, which would represent a geographical allocation of human and material resources and which would enable people to have optimum access to community nursing care. The establishment of the community nursing network should take into consideration the number of inhabitants in the region, the demographic and social structure of the population, the degree of urbanization and the region’s economic prospects. A proposal for considering the degree of rurality in the establishment of the network is also presented in the master’s thesis, as well as a calculation of the number of community health nurses that would be required in one of the regions.
