Abstract
Background Our goal was to determine the actual amount of lumbar punctures andspectrum of analyses, particularly on enzyme diagnostics, ordered at the University Clinical Center Maribor in year 2006. As the brain is very rich on glutamate dehydrogenaze (GLDH) we also attempted to discover, if there is measurable GLDH activity in cerebrospinal fluid as there is no reliable data. Brain cellular damage involves enzyme release and increases their activity in liquor in the same way as the damages of other tissues causes the enzyme release in extracellular fluids. As the haemathoencephalic barrier is normallynonpermeable for enzymes in blood does their activity in cerebrospinalfluid (CSF) mostly not derive from it. Results There were obtained 1241 lumbar punctures. Proteins and glucose were determined in all CSF samples. We did not determine any enzyme analyses in CSF. 71 CSF samples were analyzed for the presence of GLDH activity which varied form 0-340 nkat/L. Only in five CSF samples we did not find any GLDH activity. Research is still in progress. Conclusions We determined the measurable GLDH activity in the majority (92.96%) of analysed cerebrospinal fluids. This is a completely new finding. Most of analyzed CSF (32.08%) belongs to patients withmultiple sclerosis and all had measurable GLDH activity. We conclude that determining of GLDH activity in CSF is an important tool for further investigations and probably an useful diagnostic help.

Izhodišča V Univerzitetnem kliničnem centru (UKC) Maribor smo za leto 2006 hoteli ugotoviti število punkcij in spekter naročenih preiskav s poudarkom na encimski diagnostiki v likvorju. Možgani so zelo bogati z glutamat-dehidrogenazo (GLDH). Ker v literaturi ni podatkov o njegovi aktivnosti v likvorju, smo poskušali ugotoviti, ali obstajajo merljive aktivnosti GLDH. Poškodbe celic centralnega živčnega sistema (CŽS) povzročijo sproščanje encimov in dvig njihove aktivnosti v likvorju enako kot povzročijo poškodbe drugih tkiv sproščanje encimov v zunajcelično tekočino. Krvno-likvorska pregrada je normalno neprepustna za številne encime krvi, zatougotovljene encimske aktivnosti v likvorju večinoma ne izvirajo iz krvi. GLDH je parenhimski encim in se nahaja v mitohondrijih vseh celic, v možganih pa v celicah nevronov in glije. Njegova znižana aktivnost v možganih lahko privede do degenerativnih okvar oziroma bolezni. Bolniki in metode V UKC Maribor je bilo opravljenih 1241 lumbalnih punkcij. Določanje beljakovin in glukoze je bilo opravljeno v vseh vzorcih. Izvršene ni bilo nobene encimske preiskave v likvorju. Do sedaj smo na aktivnost GLDH analizirali 71 likvorjev.Encimske aktivnosti so variirale od 0-340 nkat/L. Samo v petih primerih nismo ugotovili aktivnosti GLDH. Raziskava še poteka. Rezultati V večini analiziranih likvorjev (92,96%) smo ugotovili merljive aktivnosti GLDH.Največ (32,08%) jih je bilo od oseb, obolelih z multiplo sklerozo in pri vseh smo ugotovili merljivo aktivnost GLDH. Podatek še ni objavljen v strokovni literaturi. Zaključki Ugotovitev merljivih aktivnosti GLDH v likvorju je pomembna glede na njegovo specifično vlogo v možganih. Nadaljnje raziskave bodo ugotovile, ali so tudi diagnostično značilne za določene bolezni CŽS. |
