Povzetek
Uvod in namen: Nihajna poškodba vratu je pogosta posledica prometne nesreče, v kateri pride do trka od zadaj ali od strani. Prekomerno gibanje glave in cervikalne hrbtenice naprej in nazaj lahko privede do poškodb vratnih struktur, ob čemer se pojavijo simptomi, kot so bolečina v vratu, vrtoglavica, mravljinčenje, mišična napetost, itd. Stanje lahko iz akutnega preide v kronično, kar se zgodi pri polovici poškodovancev. Namen diplomskega dela je predstaviti strukturne spremembe in spremembe v mehanskih lastnostih cervikalne hrbtenice po nihajni poškodbi. Metode: S pomočjo podatkovnih baz so bili pridobljeni znanstveni in strokovni članki, objavljeni v slovenskem in angleškem jeziku med letoma 2009 in 2019. Rezultati: Podatki iz člankov kažejo, da so po nihajni poškodbi vratu prisotni akutni in kronični simptomi ter znaki, ki se med seboj razlikujejo. Pri poškodbi pride do strukturnih sprememb mišic (povečana infiltracija maščobe, povečan presek mišice, zmanjšana gibljivost, utrujenost in zmanjšana aktivacija mišice), ligamentov (raztrganine) in fasetnih sklepov (poškodba sklepne kapsule). Uporabnost: Diplomsko delo je namenjeno fizioterapevtom pri praktičnem delu s poškodovanci po nihajni poškodbi vratu, ki želijo razumeti vzroke in posledice omenjene poškodbe. Omejitve: Pri izbiri člankov smo bili omejeni s starostjo člankov, z neustreznostjo kontrolnih skupin, s slabo kvaliteto člankov, z neenakimi kriteriji pri izbiri vzorca in s premajhnimi vzorci. Podatke imamo le od kadavrov in pedikcijskih modelov.

Introduction and purpose: Whiplash injury is considered to be a frequent consequence of a traffic accident where a collision occurs from behind or from the side. Excessive movement of the head and cervical spine back and forth can lead to injuries of the neck structures, where symptoms such as neck pain, vertigo, tingling, muscular tension, etc. can occur. The condition can go from acute to chronic, with chronic problems being reported in more than half of all injured subjects. The purpose of the graduation thesis is to present the structural changes and biomechanics of the cervical spine after the whiplash. Methods: Literature published in Slovenian and English from 2009 to 2019 was acquired in with the help of databases. Results: Scientific and professional articles report different mechanisms of whiplash injury and injured structures that could consequently lead to chronic problems. Injuries result in structural changes in the muscles (increased fat infiltration, increased muscle cross section, decreased flexibility, fatigue and decreased muscle activation), ligaments (lacerations) and faset joints (injuries to the joint capsule). Usability: The graduation thesis are intended for physiotherapists working in research and clinical practice with patients after whiplash associated disorder. Presented data could help clinicians to better understand mechanisms underlying the pathology which could enable them to better plan rehabilitation in practical work with persons after a whiplash. |
