Povzetek
Težave s kolki s staranjem prebivalstva naraščajo in potrebe po menjavi kolka se vsako leto povečujejo in bodo po predvidevanjih še naraščale, zato je potrebno najti način, da bomo tem potrebam zadostili. Z uvedbo pospešene rehabilitacije v ortopedijo, se je skrajšal čas bivanja v bolnišnici in pacienti so bili hitreje samostojni pri opravljanju vsakodnevnih aktivnosti. Pokazal se je pozitiven učinek predoperativne vadbe in psihične priprave pacienta na operacijo in potek rehabilitacije. Predoperativna vadba lahko tudi ortopedu da globlji vpogled v stanje pacienta in pomembne informacije za fizioterapevta. Po operativnem posegu je potrebno pacienta čim prej postaviti na noge, če to dopušča njegovo stanje. Naloga fizioterapevta je, da oceni stanje pacienta in na podlagi le-tega izdela načrt rehabilitacije. Pacient potrebuje v začetni fazi stalen nadzor nad izvedbo vaj; pozno fazo rehabilitacijo lahko izvaja samostojno. Rehabilitacija poteka približno eno leto in pacienti so v večini primerov z njo zadovoljni, čeprav so študije pokazale, da le v redkih primerih dosežejo polno gibljivost v kolku. Fizioterapevtska obravnava pacienta po TEP kolka je zelo različna po državah in ustanovah. V uporabi je veliko metod in tehnik, ki pa niso nujno znanstveno potrjene in preverjene; vprašljiva je tudi njihova učinkovitost. Iz pregledanih raziskav lahko zaključimo, da je učinkovita predoperativna vadba in edukacija, ki pa mora biti organizirana več kot le dan pred operacijo. Takoj po operaciji se priporoča vadbo za moč in gibljivost, vadbo v bazenu in na sobnem kolesu ter učenje pravilne hoje, v pozni fazi rehabilitaciji pa funkcionalne vaje. Za učinkovitejši pristop k pacientom po TEP kolka je potrebno opraviti dodatne raziskave, ki bodo potrdile ali ovrgle dosedanja priporočila.

The need for hip replacement increases every year and is projected to increase further. With the introduction of accelerated rehabilitation in orthopedics, the length of hospital stay was shortened and patients were more quickly independent in performing daily living activities. The positive effect of preoperative exercise and mental preparation of the patient on the operation and the course of rehabilitation was shown. Preoperative exercise can give a deeper insight into the patient’s condition and important information for the physiotherapist. After the operation, it is necessary to put the patient on his feet as soon as possible if the patient's condition allows. The task of the physiotherapist is to assess the patient's condition and make a rehabilitation plan based on his condition. In the initial phase, he needs constant supervision over the implementation of exercises and the late phase of rehabilitation can be performed by the patient independently. Rehabilitation lasts about a year and patients are satisfied with the rehabilitation in most cases. From a professional point of view, however, they rarely achieve full mobility in the hip. Physiotherapeutic treatment of a patient after hip TEP varies greatly across countries and institutions. There are many methods and techniques in use but they are not necessarily scientifically validated and tested and their effectiveness is questionable. From the reviewed research, we can conclude that preoperative exercise and education are effective. However, they must be organized more than just the day before the operation. Immediately after the operation, it is recommended to exercise for strength and mobility, exercise in the pool and on a stationary bike, and learn proper walking. In the late phase of rehabilitation, however, functional exercises. For a more effective approach to patients after hip TEP, it is necessary to conduct additional research that will confirm or refute the current recommendations. |
