Povzetek

Natrganine rotatorne manšete (NRM) so eden izmed najpogostejših vzrokov bolečine v rami. Prevalenca NRM znaša v splošni populaciji 22 %, kar 2/3 pa naj bi bilo asimptomatskih (ASIM). Namen diplomske naloge je pojasniti razlike med SIM in ASIM NRM in izdelati priporočila za pravilno obravnavo pacientov s sumom na NRM. Metode: Diplomska naloga ima obliko pregleda literature. Uporabili smo podatkovne baze NCBI (PubMed), PEDro in Cochrane Library. Rezultati: V splošni populaciji pojavnost ASIM NRM večja od SIM NRM. Poleg tega so za paciente z ASIM NRM značilni prizadetost nedominantne roke, normalna mišična moč in gibljivost, negativen znak utesnitve in velikost natrganine < 1,5 cm. Zdravljenje ni indicirano, svetujemo pa redne kontrolne preglede ter edukacijo o simptomih in dejavnikih tveganja. Uporabnost: Razvrščanje pacientov v tri skupine je uporabno v kliničnem okolju pri izbiri zdravljenja pacientov z NRM. Omejitve: Primanjkuje kakovostnih študij in kliničnih smernic za obravnavo pacientov z NRM.

Sekundarni povzetek: Rotator cuff tears (RCT) are most common cause of shoulder pain. Prevalence of RCT in general population is 22% of which 2/3 is asymptomatic (ASYM). Risk factors associated with development of RCT are age, male gender and arm dominance, history of shoulder injury, smoking habit and type II diabetes. This thesis aim to explain the difference between ASYM and SYM RCT. Goal of the thesis is to provide a reference model to guide clinical reasoning process and decision making in treatment of patients with possible RCT. Methods: Graduation thesis is literature review with following inclusion criteria used: difference between ASYM and SYM RCT, systematic reviews, clinical studies, randomized controled studies, metaanalysis, cross sectional and prospective studies, published between 2010 and 2021 in English language and diagnosis confirmed with magnetic resonance imaging (MRI) and/or ultrasonography (US) and/or at least one positive provocative test. Results: After assessment of relevant studies, we found that there is greater incidence of ASYM RCT than SYM RCT in general population. Patients with ASYM RTC have RTC more often in non-dominant arm, with normal muscle strength and range of motion, with negative impingement sign and RCT <1,5cm. Treatment is not indicated. We advise further control visits and patient education about symptoms and risk factors regarding RCT. Usability: Classification into three groups is helpful in clinical practice, when choosing the right treatment regime fo patients with RTC Limitations: More quality research is needed to provide better clinical guidelines in RTC treatment.
