Povzetek
Uvod: Pri pacientih v enoti intenzivne terapije (EIT) je stresna hiperglikemija pogost pojav. V EIT se izvajata redno spremljanje ravni glukoze v krvi pri kritično bolnih pacientih in prilagajanje odmerkov inzulina glede na meritve. Namen zaključnega dela je raziskati, kateri pristopi najučinkoviteje pripomorejo k obvladovanju stresne hiperglikemije pri kritično bolnih v EIT.
Metode: V zaključnem delu smo uporabili deskriptivno metodo raziskovanja. Opravili smo pregled znanstvene in strokovne literature, ki smo jo iskali v podatkovnih bazah CINAHL Ultimate, PubMed in SAGE Journals. Ugotovljene raziskave smo razvrstili po hierarhični strukturi, jih analizirali in predstavili v evalvacijski tabeli. Na koncu smo izvedli sintezo zbranih raziskav.
Rezultati: Identificirali smo različne glikemične kontrole, tehnologije za kontinuirano merjenje glukoze in računalniško vodene sisteme. Ugotovili smo, da še vedno ni znano, kateri pristopi najučinkoviteje pripomorejo k obvladovanju stresne hiperglikemije pri kritično bolnih v EIT.
Razprava in zaključek: Vsak posamezni pristop ima svoje prednosti in slabosti. Iz rezultatov se razkrivajo ključne dileme, kot so izbira optimalnega ciljnega glikemičnega območja, katera tehnologija za merjenje glukoze je najprimernejša in vloga digitalnih sistemov pri vodenju glikemičnega nadzora.

Introduction: Stress hyperglycaemia is common in ICU patients. In the ICU, regular monitoring of glucose levels in critically ill patients is performed and insulin doses are adjusted according to the measurements. The aim of this final work is to investigate which approaches are most effective in managing stress hyperglycaemia in critically ill patients in the ICU.
Methods: We used a descriptive research method. We conducted a review of the scientific and professional literature, which was searched in the CINAHL Ultimate, PubMed and SAGE Journals databases. The identified studies were classified according to a hierarchical structure, analysed and presented in an evaluation table. Finally, a synthesis of the collected studies was performed.
Results: We have identified different glycaemic controls, continuous glucose measurement technologies and computer-controlled systems. We found that it is still not known which approaches are most effective in helping to manage stress hyperglycaemia in critically ill patients in the ICU.
Discussion and conclusion: Each approach has its advantages and disadvantages. The results reveal key dilemmas such as the choice of the optimal target glycaemic range, which glucose measurement technology is most appropriate, and the role of digital systems in managing glycaemic control. |
