Povzetek

Uvod. Terapevtska hipotermija je zdravljenje bolnikov z namenom zmanjšanja temperature telesa pod normalno, da bi s tem izboljšali rezultate zdravljenja. Medtem ko je povprečna temperatura telesa okoli 37°C, s terapevtsko hipotermijo znižamo to temperaturo na 32°C – 34°C. Namen diplomskega dela je ugotoviti, katera biomedicinska tehnologija se uporablja na področju sistemskega hlajenja telesa in katera na področju selektivnega hlajenja možganov, ter katere od teh v Sloveniji. Metode. Pri izdelavi diplomske naloge smo uporabili deskriptivno metodo dela. Terapevtsko hipotermijo smo teoretično predstavili s pomočjo domače in tuje literature, ki smo jo iskali preko iskalnika GOOGLE, znanstveno podatkovnih baz PubMed, prosto dostopnega SCIRUS-a in virov Univerzitetne knjižnice Maribor. Za področje zdravstvene nege smo uporabili bibliografsko bazo CINAHL. Strokovne članke v slovenskem prostoru smo iskali preko servisa Biomedicina Slovenica. Rezultati. Glede na ključne besede smo v znanstveno-podatkovnih bazah našli 56235 zadetkov. V ožjem izboru smo analizirali 179 primarnih strokovnih virov, od katerih smo jih 76 uporabili v diplomskem delu. Biomedicinska tehnologija na področju terapevtske hipotermije zajema široko področje, ugotavljamo pa, da je največ tehnologij na voljo na področju sistemskega hlajenja telesa. Cenovno najugodnejša možnost je hlajenje s pomočjo ledeno hladne intravenske infuzije NaCl v kombinaciji z ledenimi oblogami ali hladilnimi ploščicami EMCOOLS. Če želimo boljši nadzor nad temperaturo telesa in samodejno uravnavanje le-te s strani aparata, pa je najboljša možnost uporaba neinvazivnega sistema CritiCool ali pa uporaba invazivnega sistema Thermogard XP s hladilnimi katetri. Za področje selektivnega hlajenja možganov bi priporočali uporabo neinvazivnega sistema RhinoChill pri odraslih, oziroma uporabo neinvazivnega sistema Olympic Cool-Cap pri otrocih. Ugotovili smo, da je uporaba terapevtske hipotermije najbolj razširjena predvsem pri bolnikih po zastoju srca in pri otrocih s hipoksično-ishemično encefalopatijo. Diskusija. Ugotovili smo, da ima terapevtska hipotermija kar nekaj pozitivnih učinkov na telo in možgane, ki bistveno izboljšajo bolnikove možnosti za preživetje in ugoden nevrološki izid, tako pri odraslih kot pri otrocih. Prišli smo do zaključka, da je terapevtska hipotermija v izkušenih rokah izredno koristna, relativno varna in zelo učinkovita.

Sekundarni povzetek: Introduction. Therapeutic Hypothermia is the treatment of patients by intentionally reducing one’s internal core body temperature below normal ranges in order to improve patient outcome. While an average internal body temperature is about 37°C, therapeutic hypothermia attempts to lower this temperature to between 32°C - 34°C. The purpose of this thesis is to determine which biomedical technology is used for the systemic cooling of body as well as selective brain cooling and which of these technologies are used in Slovenia. Methods. In the writing of this diploma thesis a descriptive method of work was used. Therapeutic hypothermia was theoretically presented by native and foreign literature, which was searched through the Google search engine, the scientific databases PubMed, freely accessible SCIRUS and the resources of the University Maribor Library. For analysing the health care we used the bibliographic database CINAHL. For analysing professional articles accessible in the Slovenian area we searched through the service Biomedicina Slovenica. Results. According to the keywords, we found 56235 hits in the scientific databases. In the final selection, we analyzed 179 professional articles, 76 of them were used in the thesis. Biomedical technology covers a wide area in the field of therapeutic hypothermia covers a wide area, but it is established that most of the technologies are available in the field of the systemic cooling of the body. The most affordable option of cooling the body is intravenous infusion of cold saline (4°C) in combination with covering the body with ice packs or EMCOOLS cooling pads. If we want better control over the body temperature and automatic regulation of it from the device, it is the best option to use the non-invasive cooling system CritiCool or the invasive system Thermogard XP with cooling catheters. In the field of selective brain cooling it is recommended to use the non-invasive cooling system RhinoChill when treating adults and the non-invasive system Olympic Cool-Cap when treating children. We established that the therapeutic hypothermia is most commonly used when treating the patients after cardiac arrest and the children with hypoxic-ishemic encephalopathy. Discussion. We established that therapeutic hypothermia has several positive effects on the body and brain, which significantly improves patient's chances of survival and gives better neurological outcome, both with adults and children. We came to conclusion that therapeutic hypothermia is extremely useful, relatively safe and very effective, when performed by experienced hands.
