Abstract

Teoretična izhodišča: Aparati za monitoring vitalnih funkcij imajo različne možnosti nastavitev alarmov in različne čase proženja alarmov. Primarni namen študije je bil raziskati, na kak način lahko prožimo alarme pri zdravih preiskovancih in kako hitro se aktivira alarm. Metodologija: Eksperimentalno raziskavo smo izvedli v kontroliranem okolju Simulacijskega centra z defibrilatorjem Lifepak 12 proizvajalca Medtronic. Alarme smo prožili po 5 krat v vsakem poskusu na simulatorju ali na prostovoljcu, ki se je fizično obremenil s počepi. Neinvazivno smo merili krvni tlak z večkratnimi zaporednimi meritvami. Nastavitve alarmov na defibrilatorju sta zajemali dve veliki skupini: avtomatska hitra nastavitev alarmov in nastavitev ozkega področja proženja alarma. Rezultati: Alarm mediane pulza se je pri simulaciji s 60 počepi prožil v 19 sekundah, v primeru 30 počepov pa v 31 sekundah. Pri večkratnih zaporednih meritvah krvnega tlaka je prišlo do proženja alarmov pri 4. in 5. zaporedni meritvi. Pri simulacijah frekvence dihanja smo ugotovili, da je lažje simulirati hitra odstopanja v dihanju ter da monitor apneo vedno zazna po 30 sekundah, oteženo je prožiti alarm PETCO2. Ugotavljamo, da alarma ventrikularne fibrilacije ni mogoče prožiti z zunanjimi ročkami, temveč s samolepilnimi elektrodami. Diskusija in zaključek: Alarmi se ob večjih odstopanjih prožijo hitreje, ob manjših odstopanjih pa počasneje. Opozarjamo, da je potrebno ob zagonu defibrilatorja aktivirati alarme, ki so sicer onemogočeni.

Secondary title: Simulations of vital function alarms on defibrillator
Secondary abstract: Introduction: Vital function control devices have different options for setting alarms at different times of alarm activation. The primary purpose of the study was to investigate how to activate alarms in healthy subjects and how quickly the alarm is triggered. Methodology: Experimental research was carried out in the controlled environment of the Simulation Center with defibrillator Lifepak 12 manufacturers Medtronic. Alarms were activated 5 times in each experiment on the simulator or at the volunteer, which was physically loaded with squats. Non-invasive blood pressure was measured with multiple successive measurements. The alarm settings on the defibrillator included two large groups: automatic alarm setup and narrow alarm zone setting. Results: Median of alarms of pulses was triggered in 19 seconds in simulation with 60 squats and in 31 seconds with 30 squats. In multiple successive measurements of blood pressure, the alarms were released in the 4th and 5th consecutive measurements. In respiratory rate simulation we found out that is easier to trigger alarms with high frequencies and that apnea is always detected at 30 seconds. It is difficult to activate the PETCO2 alarm. Discussion: Alarms are triggered more rapidly at large deviations, and with minor deviations slower. Note that when the defibrillator is turned on, the disabled alarms need to be activated.
