Abstract

Izhodišča: Suprapubični kateter je način odvajanja urina iz sečnega mehurja, za katerega se zdravniki odločijo, kadar imajo pacienti težave s samostojnim odvajanjem urina. Ključnega pomena je, da pacienti ali njihovi svojci znajo pravilno skrbeti za kateter, saj je zanj značilno, da lahko z nepravilnim rokovanjem vodi do številnih zapletov. Namen diplomskega dela je opisati pravilno zdravstveno nego pacienta s suprapubičnim katetrom. Raziskovalne metodologije in metode: Uporabili smo kvalitativno metodo raziskovanja. Izvedli smo študijo primera, za katero smo uporabili delno strukturiran intervju v katerega je bil vključen samo en pacient. Uporabili smo deskriptivno, metodo dela. Rezultati: Rezultati so pokazali, da je naš pacient tetraplegik, ter potrebuje pomoč pri vseh življenjskih aktivnostih. S študijo primera pacienta s vstavljenim suprapubičnim katetrom smo postavili naslednje negovalne diagnoze: Urin, popolna inkontinenca; koža, nevarnost za poškodovanje kože, nevarnost za spremembo kože; samonega, zmanjšana zmožnost za samostojno osebno higieno 3. stopnje; nevarnost infekcije (PNS, suprapubični kateter); Strah ( pred urološkimi zapleti in strah pred odpovedjo ledvic, oziroma dializo). Pri pacientu smo ugotovili, da ima s strani osebnega zdravnika predpisane redne obiske patronažne medicinske sestre, ki skrbi za pravilno uporabo suprapubičnega katetra. Diskusija in zaključek: Vstavljen suprapubični kateter ima pri pacientu vpliv na vse življenjske aktivnosti, ter posledično tudi na kvaliteto življenja. Ker gre v zaključnem delu za pacienta tetraplegika imajo tukaj pomembno vlogo svojci in medicinska sestra, ki mu pomagajo pri negi, ter psihofizičnem zdravju.

Secondary abstract: Introduction: Suprapubic catheter is a method of urine discharge for which physicians decide when a patient has trouble with independent urination. It is important that the patients and/or their relatives know how to handle the catheter properly otherwise it might lead to several complications. The purpose of this research is to describe an adequate health care of a patient with a suprapubic catheter. Methods: We have used the qualitative method of research. We carried out a case study for which we used partly structured interview in which one patient was included. We have used the descriptive method. Results: Results have shown that the patient was a tetraplegic who needs help with all of the everyday activities. Through the case study of a patient with inserted suprapubic catheter we have identified several nursing diagnoses: Total urinary incontinence; risk for impaired skin integrity; total selfcare deficit (3rd degree); risk for an infection (suprapubic catheter); fear (of urological complications, kidney failure and dialysis). We have discovered that the patients personal physician has deployed a community nurse whose task is to take care of a patients suprapubic catheter in his/hers home environment. Discussion: Suprapubic catheter has an important impact on a patients everyday activities and consequently on his/hers quality of life. The case study in this research was about a tetraplegic and we have come to a conclusion that the patients relatives and a community nurse have a big role in maintaining his/hers physical and mental health.
