Abstract

Uvod: Ahalazija požiralnika je resna redka motorična motnja požiralnika, ki bolniku bistveno poslabša kvaliteto življenja. Bolezni, z do sedaj poznanimi postopki, ne moremo popolnoma ozdraviti, lahko pa zdravimo oz. lajšamo njene simptome. Peroralna endoskopska miotomija je novejši kirurški poseg za zdravljenje ahalazije požiralnika, ki lahko bistveno izboljša kakovost življenja bolnika. V diplomskem delu želimo predstaviti kakovost življenja pacienta z ahalazijo požiralnika po peroralni endoskopski miotomiji. Metode: V zaključnem delu smo uporabili kvalitativno metodologijo raziskovanja. Izvedli smo intervju in izdelali študijo primera bolnika z ahalazijo požiralnika, pri katerem se je kot metoda zdravljenja uporabila peroralna endoskopska miotomija. Rezultati: Iz rezultatov je razvidno, da se je bolnikova kakovost življenja po peroralni endoskopski miotomiji bistveno izboljšala. Popolnoma se je izboljšala zmožnost prehranjevanja, posledično so se izboljšali tudi vitalnost, socialno življenje, čustveno in duševno stanje bolnika. Bolnik ni imel več večjih težav z gastroezofagealnim refluksom. Ugotavljamo pa, da kronična oblika bolezni pušča nekatere probleme, ki jih sam poseg ne reši. Razprava in sklep: Peroralna endoskopska miotomija zmanjša oz. ozdravi simptome ahalazije požiralnika, s čimer izboljša kakovost življenja bolnika, vendar ne pozdravi izvora bolezni, zato je pomembno nadaljnje pridobivanje znanja na tem področju. Pomembno je, da so zdravstveni delavci seznanjeni s problematiko ahalazije požiralnika, saj lahko tako bolnik pride do čim hitrejše diagnoze in zdravljenja.

Secondary title: Quality of life in achalasia patient after peroral endoscopic myotomy
Secondary abstract: Introduction: Esophageal achalasia is a rare severe esophageal motility disorder that significantly impairs a patient's quality of life. With the procedures known so far, the disease can not be cured entirely, but we can treat the symptoms. Peroral endoscopic myotomy (POEM) is a newer surgical procedure for treating esophageal achalasia that can significantly improve a patient's quality of life. In this thesis, we want to present achalasia and POEM as a method of treatment. Methods: In this thesis, we used a qualitative research methodology. We performed a case study on a patient with esophageal achalasia in whom POEM was used as a method of treatment. Results: The results show that the patient's quality of life after POEM improved significantly. The ability to eat has completely improved, and consequently, the vitality, social life, emotional and mental state of the patient has also improved. The patient had no gastroesophageal reflux problems after POEM. However, we find that the disease's chronic form leaves behind some problems that the POEM does not solve. Discussion and conclusion: POEM reduces or treats the symptoms of esophageal achalasia, improves the quality of life of the patient, but does not cure the origin of the disease, so it is crucial to further gain knowledge in this area. Healthcare professionals must be aware of the problem of esophageal achalasia, as this allows the patient to be diagnosed and treated as quickly as possible.
