Abstract

Teoretično izhodišče: Kronična ledvična bolezen je odpovedovanje delovanja ledvic, ki lahko napreduje v končno ledvično odpoved. Paliativna oskrba je usmerjena predvsem v oskrbo pacientov z napredovalno kronično boleznijo. Obravnava vse človeške potrebe in temelji na timskem pristopu ter dobri koordinaciji med izvajalci. Metodologija: Izvedli smo pregled in analizo literature. Literaturo smo iskali v podatkovnih bazah COBISS, PubMed, ScienceDirect in CINAHL s pomočjo ključnih besed v angleškem jeziku. Sam potek iskanja člankov smo predstavili v PRISMA diagramu. V analizo smo vključili 11 člankov, ki smo jih razdelili v dve kategoriji: vloga medicinske sestre pri paliativnem pacientu s končno ledvično odpovedjo in intervencije medicinske sestre pri paliativnem pacientu s končno ledvično odpovedjo. Rezultati: Ugotovili smo, da medicinska sestra za izvajanje paliativne oskrbe potrebuje znanje, strokovnost in čas. Intervencije, ki jih izvaja medicinska sestra pri pacientu, zajemajo obvladovanje, nadzor in zmanjševanje simptomov, bolečine, ustrezno komunikacijo, nudenje čustvene podpore tako pacientu kot družinskim članom. Razprava in sklep: Medicinska sestra mora pri pacientu opazovati simptome in jih ustrezno obvladovati oziroma zmanjševati. S pacientom se mora pogovoriti in odkriti njihove želje ob smrti. V obravnavo mora vključiti njegove družinske člane. Za paliativno oskrbo mora imeti medicinska sestra veliko znanja o bolezni ter paliativni oskrbi, ki pa mora biti za vsakega posameznika vnaprej skrbno načrtovana.

Secondary title: Palliative care of a patient with end-stage renal disease
Secondary abstract: Introduction: Chronic kidney disease represents the failure of renal functions, which can progress to end-stage renal failure. Palliative care is focused primarily on the care of patients with advanced chronic disease. It addresses all human needs and is based on a team approach and good coordination between nursing-care providers. Methods: A review and analysis of the literature was done. The search for the literature was carried out through COBISS, PubMed, ScienceDirect and CINAHL databases using keywords in the English language. The search process for articles was presented in the PRISMA diagram. 11 articles were included in the analysis, which were divided into two categories: the role of the nurse in palliative care of a patient with end-stage renal disease and the interventions of the nurse in palliative care of a patient with end-stage renal disease. Results: It was found that a nurse needs knowledge, expertise, and time to perform palliative care. Interventions performed by the nurse on the patient include management, control and reduction of symptoms, pain, and appropriate communication, providing emotional support to both the patient and family members. Discussion and conclusion: The nurse should observe the patient's symptoms and manage or reduce them accordingly. The nurse must talk to the patient and discover their wishes at the time of death. She must include the patient’s family members in the treatment. For palliative care, the nurse must have a lot of knowledge about the disease and palliative care, which must be carefully planned for each individual in advance.
