Abstract

V teoretičnem delu diplomskega dela je predstavljeno srčno popuščanje, dejavniki tveganja in zdravstvena nega. Srčno popuščanje je v razvitem svetu velik medicinski problem in je med boleznimi srca in ožilja edino bolezensko stanje, ki narašča po pojavnosti in po pogostosti. Srčno popuščanje je prisotno približno pri vsakem tretjem starostniku. Empirični del naloge opredeljuje študijo primera, v katero smo vključili 78-letnega moškega, ki je zbolel za srčno popuščanje, in njegove svojce, s katerimi smo izvedli intervju. Uporabili smo teoretično podlago tipologije vzorcev funkcionalnega zdravja po avtorici Marjory Gordon. Pridobljene podatke smo analizirali z uporabo problemskega pristopa k reševanju negovalnih problemov. Ugotovljene negovalne diagnoze smo razvrstili po posameznih vzorcih in izdelali načrt zdravstvene nege. Z raziskavo smo ugotovili, da pacient lahko živi kakovostno v svojem domačem okolju ob ustrezni podpori svojcev in rednem spremljanju njegovega zdravstvenega stanja v sodelovanju z zdravniki in z drugimi zdravstvenimi delavci. V raziskavi smo poudarili pomen vloge medicinske sestre glede zdravstveno-vzgojnih aktivnosti pri delu s pacientom in svojci. Pri tem je pomembna podpora posameznika pri vzpostavljanju zdravega življenjskega sloga, obvladovanje bolezni, samoopazovanje. Ob pomoči medicinske sestre pacient vse bolj prevzame odgovornost za lastno zdravje, saj mu ta pomaga pri oblikovanju kritičnega odnosa do bolezni in zdravja kot pomembne vrednote.

Secondary abstract: In the theoretical part the diploma work deals with heart failure, risk factors and nursing care. In the developed parts of the world heart failure presents a serious medical problem, being today the only cardiovasculare disease that increases both in prevalence and incidence. It is estimated that heart failure occurs in every third elderly person. The empirical part defines the study of a case involving a 78 years old man with heart failure and his relatives who have been interviewwd. We have used the theoretical typology of functional health patterns defined by Marjory Gordon. The data have been analysed with the use of the problematic approach to solving nursing problems. The gained nursing diagnoses have been classified according to individual sampes and a nursing plan has been made. The research has show that the patient can have a normal life home environment with the proper support from the relatives and regular monitoring of the patients health state with the cooperation with the physician and other healthcare workers. The research has emphasized the meaning of the nurses role in the sense of healt and education activities with the patient and relatives. The support with the healthy lifestyle of the patient is very important as well as gaining control of the diseases, self – monitoring and measuring of his/her own health because the nurse helps modelling a critical approach to the disease and health as a vital commodity.
