Abstract
Uvod: Pooperativni delirij je nespecifični organski možganski sindrom, ki se kaže v motnjah zavesti, pozornosti, zaznavanja, mišljenja, čustvovanja in spominjanja. Po operaciji na srcu se lahko razvije v nekaj urah pa do nekaj dni po operaciji. Pri bolnikih se pooperativni delirij po operaciji srca pojavlja med 5 % in 55 %. Namen zaključnega dela je bil izpostaviti pomen zgodnjega prepoznavanja delirija pri bolnikih po operaciji na odprtem srcu.
Raziskovalne metode: Opravili smo sistematični pregled in analizo znanstvene in strokovne literature. Literaturo smo s pomočjo vključitvenih in izključitvenih kriterijev, ki smo jih oblikovali iz raziskovalnega vprašanja, iskali v podatkovnih bazah MEDLINE, CINAHL IN SAGE. Potek iskanja je prikazan s pomočjo PRISMA diagrama.
Rezultati: V končni pregled smo vključili 8 člankov. Najpogostejše intervencije za zgodnje prepoznavanje pooperativnega delirija so izobraževanje medicinskih sester in pacientov, opazovanje in ocenitev bolnikov po lestvicah za zgodnje prepoznavanje delirija, vključitev družinskih negovalcev, obvladovanje bolečine, izboljšanje kvalitete spanja, normaliziranje elektrolitov in vnos tekočin ter orientacija bolnikov.
Razprava in sklep: Za preprečevanje pooperativnega delirija po operaciji na odprtem srcu so intervencije medicinskih sester za zgodnje prepoznavanje znakov delirija učinkovite. Z razvojem pooperativnega delirija se poveča tveganje za različne zaplete in smrt, podaljša se čas zdravljenja, višji so finančni stroški, zato je izjemno pomembno, da se o prepoznavanju pooperativnega delirija medicinske sestre dodatno izobrazi o dejavnikih tveganja, znakih, uporabi ocenjevalnih lestvic in intervencijah za medicinske sestre, ki se vsakodnevno srečujejo s pooperativnim delirijem po operaciji srca.

Early detection of prostoperative delirium in patients after open heart surgery |

Introduction: Post-operative delirium is a non-specific organic brain syndrome that manifests itself in disturbances of consciousness, attention, perception, thinking, emotion and memory. It can develop within a few hours or up to a few days after open-heart surgery. Post-operative delirium after heart surgery occurs in between 5% and 55% of patients. The purpose of the thesis was to highlight the importance of early recognition of delirium in patients after open heart surgery.
Research methods: We have carried out the systematic overview and analysis of scientific and technical literature. We searched for the appropriate literature in MEDLINE, CINAHL and SAGE databases using the inclusion and exclusion criteria that we developed from the research question. The search process is displayed with the help of PRISMA diagram.
Results: We included 8 articles in the final overview. The most common interventions for early recognition of post-operative delirium are the education of nurses and patients, observation and evaluation of patients on scales for early delirium recognition, inclusion of family caregivers, pain management, improvement of sleep quality, normalizing electrolytes and fluid intake, and patient orientation.
Discussion and conclusion: To prevent post-operative delirium after open-heart surgery, interventions by nurses for early recognition of signs of delirium are effective. With the development of post-operative delirium, the risk for various complications and death increases, the time of treatment is extended and the financial cost is higher. Thus, it is extremely important to additionally educate nurses about the recognition of post-operative delirium, about risk factors, signs, the use of assessment scales and about the interventions for nurses who are working with patients with post-operative delirium after heart surgery on a daily basis |
