    Abstract
 
Uvod: Potreba po snemno protetičnih nadomestkih je vse večja, saj se svetovno prebivalstvo stara. Pacienti z delno brezzobim statusom pa se pogosto odločajo za delno protezo s kovinsko bazo, kot protetičnim nadomestkom, saj je to za njih cenovno bolj ugodno. Namen: Namen diplomskega dela je na podlagi pregleda strokovne in znanstvene literature predstaviti načrtovanje kovinske baze pri delni protezi. Opisan in strokovno prikazan je postopek načrtovanja in izdelave kovinske baze po klasični metodi, ter dokončna izdelava delne proteze. Metode dela: V diplomskem delu smo opravili pregled domače in tuje literature v slovenskem in angleškem jeziku, na temo načrtovanja kovinske baze pri delni protezi. Glavni vir informacij je bila strokovna literatura, kot so strokovne knjige in revije ter strokovni članki. Literaturo smo našli s pomočjo knjižnic in strokovnih baz. V praktičnem delu smo v laboratoriju pod nadzorom mentorja načrtovali kovinsko bazo na podlagi že opisanih postopkov iz pregledanih strokovnih člankov ter predstavili potek izdelave delne proteze po klasični metodi. Rezultati: Izdelali smo spodnjo in zgornjo delno protezo. Uporabili smo dva načina izdelave, zgornjo kovinsko bazo smo izdelali po klasični metodi z ulivanjem kovine, spodnjo pa smo klasično načrtovali v paralelometru, nato pa modelirali računalniško in 3D natisnili. Zgornjo kovinsko bazo smo uspešno podvojili v kovini in se je dobro prilegala na izvorni mavčni model. Tudi spodnjo kovinsko bazo smo dobro zmodelirali in se je prilegala izvornemu mavčnemu modelu. Kovinski bazi smo klasično dokončali in spolirali v zobnem laboratoriju, nato pa smo nadaljevali z dokončanjem delne proteze. Na umavčena modela v artikulator smo postavili manjkajoče zobe in zatlačili akrilat. Akrilat smo po polimerizaciji obdelali z brusnimi sredstvi in spolirali na polirnem stroju. Končni izdelek se je lepo prilegal na izvorni model. Ugotovili smo, da se načrtovanje kovinske baze sklada s priporočeno prakso. Razprava in zaključek: Pacienti z delno brezzobim statusom še vedno kot protetični nadomestek raje izberejo delno protezo z ulito kovinsko bazo, saj je to cenovno bolj ugodno. Neposredni cilji protetičnega zdravljenja pri delno brezzobih pacientih, z uporabo snemne delne proteze, so namenjeni boljšemu videzu in obnovitvi delovanja ter zaščiti preostalega tkiva. Pri načrtovanju kovinske baze je potrebno upoštevati načrt, ki ga pripravi zobozdravnik. Kovinsko bazo v zobotehničnem laboratoriju se načrtuje s pomočjo paralelometra. Pri načrtovanju je potrebno upoštevati vse dejavnike, pomembno je dobro preučiti sile, ki bodo delovale na tkiva in sidrne zobe. Pomembna je tudi izbira ustreznih zapon in oblikovanje velikega veznega elementa. Izdelava delne proteze z ulito kovinsko bazo je še vedno zelo razširjena in tudi uspešna metoda za reševanje delne brezzobosti pacienta. V zadnjih letih se klasično načrtovanje kovinske baze ni veliko spreminjalo.

            Introduction: The need for removable prosthetic replacements is increasing as the world's population ages. Patients with partially edentulous status often decides for partial dentures with a metal base as a prosthetic replacement, as this is more affordable for them. Purpose: The purpose of the thesis is to present the design of a metal base for partial dentures based on a review of professional and scientific literature. The process of planning and manufacturing a metal base according to the classical method is described and expertly demonstrated, as well as the final manufacturing of a partial prosthesis. Methods: In the diploma thesis, we have carried out a review of domestic and foreign literature in Slovene and English on the topic of metal base design in partial dentures. The main source of information was scientific literature, such as technical books and journals, and technical articles. Literature was found using libraries and expert databases. In the practical part, under the supervision of a tutor, we designed a metal base in the laboratory, based on the procedures already described in the scientific articles and presented the process of fabricating a partial denture using the classical method. Results: We made a lower and an upper partial denture. We used two methods of fabrication, the upper metal base was fabricated using the classical method of metal casting and the lower base was classically designed in a parallelometer, then modelled on a computer and 3D printed. The upper metal base was successfully duplicated in metal and fitted well to the original plaster model. The lower metal base was also well modelled and fitted to the original plaster model. The metal bases were classically finished and polished in the dental laboratory and then we proceeded to finish the partial denture. We placed the missing teeth on the plastered models in the articulator and pressed in the acrylate. After polymerisation, the acrylate was treated with abrasives and polished on a polishing machine. The final product was a good fit on the original model. We found that the design of the metal base was in line with recommended practice. Discussion and conclusion: Patients with partially edentulous status still prefer a partial denture with a cast metal base as a prosthetic replacement, as it is more affordable. The immediate goals of prosthodontic treatment in partially edentulous patients using removable partial dentures are to improve appearance and restore function, and to protect the remaining tissue. When planning the metal base, the plan drawn up by the dentist must be followed. The metal base in the dental laboratory is designed using a parallelometer. All factors must be taken into account in the design, and it is important to consider the forces that will act on the tissues and anchor teeth. The choice of appropriate clasps and the design of the large abutment are also important. The fabrication of a partial denture with a cast metal base is still a widespread and also successful method to address the partial edentulousness of the patient.
