Abstract

Temeljno izhodišče: Palatalni mioklonus je bolezen, pri kateri gre za ritmično krčenje mehkega neba, zaradi česar pacienti pogosto slišijo klikanje v ušesih. Vzrok je lahko organski ali nepojasnjena funkcionalna motnja, pogosto pa je psihogenega izvora. V tem primeru govorimo o psihogenem palatalnem tremorju. Namen diplomskega dela je predstaviti palatalni mioklonus in na praktičnem primeru pacienta s palatalnim mioklonusom ugotoviti, pri katerih štirinajstih osnovnih življenjskih aktivnostih se pojavljajo težave, pri tem pa izpostaviti glavne negovalne diagnoze. Raziskovalna metodologija: Diplomsko delo vključuje praktični primer 32-letnega pacienta s psihogenim palatalnim mioklonusom v obdobju od leta 2009 do vključno decembra 2014. Pacient je bil obravnavan s pomočjo delno strukturiranega vprašalnika in zdravstvene dokumentacije. Rezultati: Na podlagi zbranih podatkov smo ugotovili, da se pri pacientu s psihogenim palatalnim mioklonusom negovalne diagnoze po Marjory Gordon pojavljo pri prvi osnovni življenjski aktivnosti (OŽA) dihanje, pri tretji OŽA izločanje in odvajanje, četrti OŽA gibanje in ustrezna lega, peti OŽA spanje in počitek in deseti OŽA komunikacija, odnosi z ljudmi, izražanje čustev, občutkov, potreb. Ugotovili smo tudi, da psihogeni palatalni mioklonus in njegovo zdravljenje negativno vplivata na socialno življenje in kakovost življenja pacienta. Diskusija in zaključek: Pri pacientu s psihogenim palatalnim miokonusom se pojavljajo psihične težave, ki se manifestirajo na telesni ravni, le-te pa spremljajo težave na vedenjskem in čustvenem področju, znižujejo zaupanje do sočloveka ter negativno vplivajo na vzpostavljanje in vzdrževanje medsebojnih odnosov. Medicinska sestra je pri obravnavi pacienta v vlogi vira informacij, svetovalke, terapevtke, učiteljice in nadomestne osebe.

Secondary abstract: Theoretical background: Patients suffering from palatal myoclonus, a disease followed by characteristic rhythmical shrinking of the soft palate, often claim to hear clicking sounds in their ears. The cause might be organic or an unexplained functional obstruction, but the disease is often of psychogenic origins. In such cases we may talk about a psychogenic palatal tremor. It is the main goal of the diploma to introduce palatal myoclonus, point out the problems occurring within fourteen basic life activities on the base of a patient suffering from palatal myoclonus, and determine all main nursing diagnosis. Research methodology: The diploma includes a case study of a 32-year-old male patient suffering from psychogenic palatal myoclonus in a time period from 2009 to December 2014. The patient was treated with help of a partly structured questionnaire and the overview of his medical records. Results: Regarding the assembled data, it has been determined, that in case of the patient suffering from psychogenic palatal myoclonus nursing diagnosis by Marjory Gordon occur within the following basic life activities (BLA): the first BLA (breathing), the third BLA (secretion and evacuation), the fourth BLA (motion and appropriate lying), the fifth BLA (sleeping and resting), and the tenth BLA (communication, relationships with people, expressing emotions, feelings, needs). Last but not least, it has been examined, that psychogenic palatal myoclonus and its treatment have a negative impact on patient's social life and its quality. Discussion and conclusion: The patient suffering from psychogenic palatal myoclonus shows psychic problems on physical level followed by problems associated with behaviour, sentiment, diminishing of trust, and negative influence on restoring and keeping up relationships. When treating such a patient, a nurse plays the role of an information source, a consultant, a therapist, a teacher, and a substitutive person.
