Povzetek
Dvojna diagnoza se pojavi pri pacientih, ki so odvisni od psihoaktivnih snovi ali alkohola in imajo hkrati pridružene še druge duševne motnje ali bolezni. Pri opredeljevanju dvojne diagnoze se postavljajo vprašanja kaj je bilo prej, odvisnost od psihoaktivnih snovi ali duševna motnja. V diplomskem delu smo predstavili obravnavo pacienta z dvojno diagnozo in težave, ki se pojavljajo pri obravnavi. Predstavili smo zdravstveno nego pacienta z dvojno diagnozo ter opisali vlogo medicinske sestre pri obravnavi. Pri pisanju smo uporabili deskriptivno metodo dela s študijem domače in tuje strokovne literature. Pacient, ki ima poleg odvisnosti od psihoaktivnih snovi tudi duševno motnjo, zahteva drugačen pristop zdravljenja in obravnave kot tisti, ki duševne motnje nima. Težave pri zdravljenju pacientov z dvojnimi diagnozami so predvsem v tem, da vsako od motenj zdravi druga institucija, te institucije pa se med seboj težje uskladijo glede načina obravnave pacientov. Pomembna vloga medicinske sestre je psihoedukacija. Ta je večinoma individualna in jo prilagodimo zmožnostim in potrebam pacientov. Z njo na poseben način poučimo pacienta na njemu razumljiv način o njegovem zdravljenju. Pacienta seznanimo z informacijami o škodljivih učinkih in možnih psihičnih, fizičnih, ekonomskih in socialnih posledicah uživanja psihoaktivnih snovi ter ga poučimo, kako na njegovo duševno motnjo vpliva zloraba psihoaktivnih snovi. Pomanjkljivo znanje na področju zdravstvene nege pacientov z dvojnimi diagnozami in nedorečenost ukrepov na samem oddelku vpliva na to, da je zdravstveno osebje v odnosu do pacienta večkrat v dilemi kako ravnati, zato bi bilo potrebno zdravstveno osebje bolje seznaniti in poučiti kako primerno ravnati s temi pacienti.

Dual diagnosis occurs in patients that are addicted to psychoactive substances or alcohol, while also suffering from other mental disorders or illnesses. When establishing a dual diagnosis the question comes up, which occurred earlier: the addiction to psychoactive substances or the disorder? The subject of the thesis is a study of a patient with dual diagnosis and the problems related to it. We presented the treatment of a patient with dual diagnosis and described the medical nurse’s role during the processing. We used a descriptive method with a study of domestic and foreign professional literature. Patients with addiction to psychoactive substances and suffering from mental disorders require a non-standard approach. The major problem in treatment of patients with dual diagnosis is that each disorder is treated by a different institution and that the institutions have difficulties in coordinating their treatments. Nurses play an important role by providing psychoeducation. This is mostly done on the individual level and can be adapted to the needs of the patient. It is used to help the patient understand his treatment. The patient is informed about the negative effects and possible physical, economic and social consequences of psychoactive substance abuse and its effects on his mental disorder. Lacking knowledge in nursing care of patients with dual diagnosis and half-way measures in the medical ward can often make the medical stuff unsure of how to properly treat the patient, therefore the medical staff would have to be better informed and trained in how to adequately treat such patients. |
