Abstract

Izhodišča: V zadnjih letih se je povečalo število bolnikov s potrebo po hranjenju po perkutani endoskopski gastrostomi (PEG). V praksi se je izkazalo, da se bolnikom po vstavitvi PEG izboljša zdravstveno stanje. Svojcem in zdravstvenemu osebju pa je olajšano hranjenje in negovanje. Pri teh bolnikih gre za težje oblike bolezni, zato se pa pojavljajo tudi zapleti v povezavi s PEG. Določene zaplete lahko s pravilnim negovanjem in strokovnim znanjem preprečimo. Metode: Raziskovali smo s pomočjo kvantitativne metodologije dela. S pomočjo strokovne literature smo predstavili teoretična izhodišča. Kot raziskovalni instrument smo uporabili dokumentacijo stotih bolnikov iz arhiva Oddelka za gastroenterologijo in endoskopijo UKC Maribor (od leta 2004 do 2014). Podatki so statistično obdelani s pomočjo računalniškega programa Microsoft Office Excel 2010, katerih rezultate smo pokazali v obliki grafov. Rezultati: Rezultati raziskave kažejo, de je med stotimi analiziranimi dokumenti bolnikov z vstavljeno PEG prišlo do zgodnji zapletov pri osmih bolnikih. Zamenjavo PEG je potrebovalo manj kot tretjina bolnikov. Pri več kot polovici bolnikov, ki so potrebovali menjavo PEG je bila izvedena po enem letu. V manj kot polovici bolnikov pa je menjava PEG bila izvedena pred enim letom. Najpogostejši vzrok za menjavo je bil obraba materiala. Drugi najpogostejši vzrok, menjave PEG pa je bil nenameren izpad sonde. Sklep: V naši raziskavi smo ugotovili, da je zgodnjih zapletov pri vstavitvi PEG malo. Več zapletov se pojavi kasneje po vstavitvi PEG, katere pa bi morda s strokovnim znanjem, pravilnim negovanjem, usposobljenim kadrom, izobraževanjem in zdravstveno vzgojo svojcev lahko preprečili.

Secondary abstract: Introduction: In the last few years, the number of patients with the need of percutaneous endoscopic gastrostomy (PEG) increases. In the practice, it shown that the insertion of PEG improved patient’s health status. In addition, it made feeding and nursing easier for the relatives or medical staff. There can also some complications occur in connection to PEG, because of the degree of the disease. Certain complications can be prevented with the proper care and expertise. Methods: We studied using quantitative methodologies. With the help of the scientific writings, we presented the theoretical foundations. As a research tool, we used the documentation from the archives of the Department of gastroenterology and endoscopy UKC Maribor (2004 to 2014). Data were statistically analysed using the computer program Microsoft Office Excel 2010, the results of which are shown in the form of graphs. Results: The results of our study show that among hundred analysed documents of patients with inserted PEG eight patients had experience complications. The substitution of PEG was needed by less than third of patients. More than half of patients who needed a PEG substitution, the substitution was made after one-year time. Less than half of patients required substitution of PEG before one-year period. The most common cause of substitution was material wear. The second most common cause, however, was inadvertent loss of the stylet. Conclusion: In this study, we found out that early complications with the insertion of PEG are rare. Complications that occur later after the insertion of PEG are more common, but they could be avoided with the expertise, proper nursing and care, skilled personnel, education/training and with the providing of proper information to the patient’s relatives.
