Povzetek

Izhodišče in namen: V diplomskem delu je obravnavana zdravstvena nega pacienta po operativnem posegu, pri katerem so izvedli ileostomo. Namen obravnave je bil ugotoviti kako pacient sprejema svoje zdravstveno stanje in kako mu je pri tem medicinska sestra v oporo. Raziskovalna metodologija in metode: V teoretičnem delu je opisana anatomija prebavne cevi, ileostoma, zapleti pri ileostomi, zdravstvena nega pacienta po operativnem posegu in zdravstveno vzgojno delo medicinske sestre po odpustu pacienta iz bolnišnice. V empiričnem delu je predstavljena študija primera pacienta po operativnem posegu. Pacienta smo obravnavali po 14 življenjskih aktivnostih Virginije Henderson in postavili negovalne diagnoze. Vse potrebne podatke smo pridobili s pomočjo intervjuja in negovalne ter medicinske dokumentacije. Rezultati: Ugotovili smo, da so negovalni problemi prisotni pri naslednjih življenjskih aktivnostih: prehranjevanje in pitje, izločanje in odvajanje, oblačenje in urejanje, čistoča in nega telesa, spanje in počitek, učenje in pridobivanje znanja o zdravju ter izogibanje nevarnosti v okolju. Negovalni problem predstavlja tudi bolečina. Velik poudarek je na učenju in pridobivanju znanja, saj medicinska sestra posreduje pacientu veliko novih informacij glede prehrane, nege stome, odvajanja, gibanja ter oblačenja. Diskusija in zaključek: Ileostoma vpliva na spremembo samopodobe. S tem ima pacient največ težav, zato je potrebno pacientu nuditi veliko psihične podpore in postopnega navajanja na trenutne spremembe.

Sekundarni povzetek: Background: In the final work is treatment nurse care of patient after a surgical procedure in which was performed ileostomy. The purpose of treatment was to find whether and how do patients accept their medical diagnosis and what kind of support does nursing team provide them. Research methodology: In the theoretical part is description of the anatomy of the digestive tube, ileostomy, complications in ileostomy, health care of patient after surgery and medical educational work of the nurse after discharge of the patient from the hospital. In the empirical part we conducted a case study on patient after surgery. We treated patient resultant of 14 life activities based on the theory of Virginia Henderson. We exposed eight most important nursing diagnosis. All necessary information were obtained through interviews and nursing and medical records. Results: We have found that the nursing problems are presented in following life activities: eating and drinking, eliminating and draining, dressing and tidying up, cleanliness and body care, sleeping and resting, learning to gain knowledge about health and avoiding environmental hazards. Pain is also a nursing problem. Large emphasis is needed on learning and gaining knowledge, because the nurse provides the patient a lot of new information regarding nutrition, stoma care, discharge, movement and dressing. Discussion and conclusion: Ileostoma affect the change in self-image. With this the patient might have a lot of problems so it is necessary to provide the patient psychic support and help to rapid adjustment to the current health changes.
