Povzetek
Uvod: Test nadzora trupa je preprosto merilno orodje za oceno premičnosti v postelji in ravnotežja v sedečem položaju. Namen pregleda literature je bil ugotoviti njegove merske lastnosti pri različnih populacijah pacientov. Metode: Pregledane so bile podatkovne zbirke PubMed, CINAHL in Cochrane. Rezultati: V pregled je bilo vključenih 17 raziskav. Pri pacientih po možganski kapi in pacientih z živčno-mišičnimi boleznimi je test nadzora trupa srednje do zmerno notranje skladen (α = 0,75–0,86), ima odlično zanesljivost posameznega preiskovalca (ICC = 0,98) in zelo dobro zanesljivost med preiskovalci (ro = 0,76). V devetih raziskavah od desetih je bila potrjena veljavnost konstrukta. V štirih raziskavah so ugotavljali hkratno veljavnost. Z napovednimi modeli, ki vsebujejo test nadzora trupa, je mogoče napovedati ravnotežje, hojo, funkcijsko samostojnost, dolžino bivanja v rehabilitacijski ustanovi in mesto odpusta. Je dobro odziven (ROC = 0,92), vendar se pogosto pojavi učinek stropa. Zaključki: Test nadzora trupa je primeren za uporabo pri ljudeh z nizko stopnjo premičnosti. Zaradi učinka stropa ni priporočen za splošno uporabo. Potrebne so nadaljnje raziskave.

Psychometric properties of the trunk control test and relationship of trunk control with improvement of functioning |

Introduction: Trunk control test is a simple measurement tool for assessment of bed mobility and sitting balance. The purpose of the literature review was to establish its psychometric properties in different patient populations. Methods: PubMed, CINAHL, and Cochrane databases were reviewed. Results: 17 studies were included. The trunk control test has fairly to moderate internal consistency (α = 0.75–0.86), excellent intra-rater reliability (ICC = 0.98), and very good inter-rater reliability (rho = 0.76) in patients after stroke and patients with neuromuscular disease. In nine out of ten studies, construct validity or known groups validity was confirmed. Concurrent validity was investigated in four studies. Prediction models which include trunk control test predict balance, walking ability, functional independence, length of stay, and the discharge outcome. This test has good responsiveness (ROC = 0.92), however a ceiling effect occurs commonly. Conclusions: The trunk control test is appropriate for use in patients with low level of mobility. Because of ceiling effect, it is not recommended for general use. Further studies are needed. |
