Povzetek
Uvod: Vadba hoje na lokomatu je eden izmed fizioterapevtskih postopkov, s katerim želimo vplivati na vzpostavitev ali izboljšanje različnih vidikov hoje pri osebah z nepopolno okvaro hrbtenjače. Namen članka je bil ugotoviti, ali je učinkovitost vadbe hoje na lokomatu večja v primerjavi z drugimi fizioterapevtskimi postopki. Metode: V sistematični pregled literature smo vključili randomizirane kontrolirane poskuse, s katerimi so preiskovali odrasle z nepopolno okvaro hrbtenjače. Pregledali smo zbirke PubMed, Cinahl, PEDro in Cochrane do januarja 2015. Rezultati: Vključitvenim merilom je ustrezalo pet raziskav, objavljenih med letoma 2005 in 2014. Dve sta obravnavali preiskovance v prvem letu po okvari in tri preiskovance več kot leto po okvari. Študije so primerjale od dva do štiri postopke, pri vseh je bil eden vadba hoje na lokomatu. Le v eni raziskavi s preiskovanci v prvem letu po začetku okvare so se vzdržljivost hoje, uporaba pripomočkov in neodvisnost med hojo ter mišična zmogljivost bolj izboljšali v skupini, ki je vadila na lokomatu. Zaključki: Dokazov, ki bi dajali prednost enemu izmed postopkov, ni, vsi so potencialno učinkoviti. Potrebno je nadaljnje raziskovanje vplivov vadbe hoje na lokomatu v primerjavi z drugimi fizioterapevtskimi postopki za vadbo hoje.

Comparison of the effects of gait training using lokomat and other physiotherapeutic procedures in patients with incomplete spinal cord injury |

Background: Gait training with lokomat is one of the physiotherapeutic procedures for gait re-education or improvement of different gait aspects in patients with incomplete spinal cord injury. The purpose of the article was to establish whether efficiency of gait training with lokomat is greater than with other physiotherapeutic procedures. Methods: Randomized controlled trials studying adults with incomplete spinal cord injury were included in the systematic review. PubMed, Cinahl, PEDro and Cochrane databases were searched until January 2015. Results: Five studies met the inclusion criteria. They were published between 2005 and 2014. Two studies included subjects within first year after injury onset, three after one year. Studies compared two to four procedures, with gait training using lokomat being one of them. Only one research with subjects within first year after injury onset showed better improvement of walking distance, usage of devices, braces and physical assistance during walking, and muscle strength in the lokomat group. Conclusions: There is no evidence on superiority of one procedure over another. All procedures show potential for efficiency. More research of the effects of gait training using lokomat in comparison with other physiotherapeutic procedures is needed. |
