Povzetek

V diplomskem delu smo predstavili ocenjevanje bolečine kot pomemben element zagotavljanja kakovostne zdravstvene nege. To je osnova pomoči, ki jo medicinske sestre nudijo bolniku z bolečino. Od pravočasnega prepoznavanja in ocenjevanja bolečine je odvisen izid zdravljenja in zadovoljstvo bolnikov, ki sta kazalnika kakovosti zdravstvene oskrbe. V diplomskem delu smo opisali fiziologijo bolečine in prikazali vrste bolečin. Posebej smo prikazali akutno pooperativno bolečino, vplive nelajšane bolečine na organizem ter predstavili službo za lajšanje pooperativne bolečine v UKC Ljubljana. Ker je doživljanje in zaznavanje bolečine zelo subjektivna izkušnja, na katero vplivajo mnogi dejavniki, ki so odločilni pri postavitvi objektivne ocene bolečine in ki vplivajo tudi na bolnikove odzive na bolečino, smo te dejavnike podrobno opisali. Predstavili smo lestvice in druga orodja za oceno in merjenje jakosti bolečine. V diplomskem delu smo predstavili tudi elemente kakovostne zdravstvene nege, ki je cilj aktivnosti medicinskih sester. Standardi so orodje za merjenje kakovosti zdravstvene nege. V diplomskem delu smo oblikovali primer vsebinskega standarda po metodologiji E. Mason, ki omogoča kakovost izvajanja komunikacij v zdravstveni negi s poudarkom, kaj bolnika naučimo in katere informacije mu posredujemo. Za raziskavo, ki je bila izvedena v UKC Ljubljana na kliničnem oddelku za abdominalno kirurgijo, smo izpostavili naslednje hipoteze: H1: Osveščenost bolnikov o lajšanju bolečine pred operacijo in poučenost o samonadzoru zmanjša doživljanje bolečine po operaciji. H2: Ocenjevanje pooperativne bolečine in samonadzor poveča zadovoljstvo bolnikov. H3: Večini bolnikov je bila pooperativna bolečina ustrezno lajšana. Metode: Raziskava je bila izvedena v času od maja 2007 do novembra 2007. V raziskavi je sodelovalo 53 bolnikov, ki so bili operirani zaradi raka na DČD. Uporabili smo anketni vprašalnik, s katerim smo želeli ugotoviti obveščenost bolnikov pred operativnim posegom o pravici do lajšanja bolečine, o možnostih in metodah lajšanja, o ocenjevanju bolečine, o pripomočkih za merjenje jakosti bolečine, vplivu na zadovoljstvo bolnikov in na kakovost zdravstvene nege. Za obdelavo podatkov smo uporabili standardne statistične metode. Kvantitativna obdelava je bila ročna. Tabele in grafikone smo uredili z računalniškim programom Microsoft Excel. Rezultati: Obveščenost in poučenost bolnikov o bolečini pred operacijo vpliva na čutenje bolečine. Bolniki so bili zelo zadovoljni z ocenjevanjem bolečine po operaciji., njihova bolečina je bila učinkovito lajšana.

Sekundarni povzetek: Assessment of pain as an important factor in assuring high quality nursing care was introduced in diploma. This is basic nursing care for patient with pain. The two health supply indicators: outcome and satisfaction with treatment and nursing care depend on timely recognizing and assessing pain. In diploma we described physiology of pain and showed types of pains, with major focus on postoperative pain and influence of untreated pain. We introduced nursing care service for relieving postoperative pain in University Medical Centre Ljubljana. Undergoing and perception of pain is very personal experience on which influence many factors crucial in objective assessment of pain. That factors also influence patient`s reactions on pain, so they are described in details. Scales and other instruments for assessment and measuring the strenght of pain are stated. In diploma we also introduced elements of quality nursing care, the goal of nursing activities. Nursing standards are instruments for measuring the quality of nursing care. In diploma we formed standard using methodology E. Mason, which allows quality communication in nursing care, teaching and offering informations to patients. In University Medical Centre Ljubljana, on Department of abdominal surgery we reserched the following hypothesies: H1: Patients awareness about relieving pain before surgery influences on strenght of pain. H2: Identifying and assessment of postoperative pain influence on higher satisfaction with care. H3: Most of patients have appropriately relieved postoperative pain. Methods: 53 patients, operated because of cancer on wide intestine and rectum, took part in research performed from May to November 2007. With questionary we wanted to find out patients knowledge about their rights of relieving pain, about possibilities and methods of relieving pain, assessments and accessories for measuring the strenght of pain and its influence on patients satisfaction with healing and on quality of nursing care. Standard statistical methods were used with manual quantitative processing. Tables and graphs were settled with computer programme Microsoft Excel. Results: Teaching patients about pain before surgery reduce postoperative pains. Patients were very satisfied with pain assessment and with relieving pain after surgery.
