Povzetek

Ob nenadnem srčnem zastoju veliko ljudi ne ve, kako pristopiti in ukrepati ob nastali situaciji, zato imajo pomembno vlogo pri obravnavi takšnih klicev zdravstveni dispečerji. Kadar dispečer v klicu v sili prepozna srčni zastoj in skuša zagotoviti navodila za oživljanje, lahko nekateri nepredvidljivi dejavniki delujejo kot ovire. Namen sistematičenega pregleda literature je bil ugotoviti ovire ob kardiopulmonalnem oživljanju podprtem z navodili dispečerja po telefonu. Za sistematičen pregled literature smo uporabili zbirke podatkov PubMed, CINAHL, ScienceDirect, Wiley Online Library, Web of Science in SAGE. Iskanje je potekalo s pomočjo ključnih besed: cardiac arrest, dispatcher assisted, bystander, cardiopulmonary resuscitation, telephone CPR ter z uporabo njihovih sopomenk in Boolova operatorja (AND/OR). V analizo smo vključili vse vire, objavljene do leta 2020 v angleškem jeziku. Iz iskalnega nabora 2143 je bilo v končno analizo vključenih 10 virov. Pri analizi virov so bili identificirane 3 glavne teme, ki opredeljujejo ovire pri kardiopulmonalnem oživljanju podprtem z navodili dispečerja po telefonu, in sicer: osebni dejavniki, postopkovne ovire in znanje. V temah smo kasneje identificirali še 23 podtem. Pacienti, ki so doživeli zunajbolnišnični srčni zastoj, prejmejo pomoč s strani najditelja oziroma klicatelja, kateri pokliče številko nujne medicinske pomoči. Delo zdravstvenega dispečerja je odgovorno ter zelo stresno. Komunikacija je glavni člen verige sodelovanja med zdravstvenimi dispečerji ter klicatelji. Kljub oviram, na katera so naleteli bodisi klicatelji ali zdravstveni dispečerji, se je v večini primerov izteklo, da se je potrebna pomoč nudila.

Sekundarni povzetek: In the event of a sudden cardiac arrest many people do not know, how to approach the effected or what actions to take in the situation, therefore medical dispatchers play an important role in dealing with such calls. When an emergency call dispatcher recognizes the situation and tries to provide resuscitation instructions, different unexpected factors can act as barriers. The purpose of the systematic review of the literature was to identify barriers to CPR resuscitation supported by telephone dispatcher instructions. We used PubMed, CINAHL, ScienceDirect, Wiley Online Library, Web of Science and SAGE databases for the systematic literature review. We conducted the search using the keywords: cardiac arrest, dispatcher assisted, bystander, telephone CPR and cardiopulmonary resuscitation, while also using their synonyms and the Boolean operator (AND/OR). In the analysis, we included all sources published by 2020 in English. From 2143 sources in the search draft, only 10 were included in the final analysis. In the analysis of the sources, we found three main topics, which identify barriers to cardiopulmonary resuscitation supported by telephone dispatcher’s instructions. These barriers were namely: personal factors, procedural barriers, and knowledge. In these topics, we later identified another 23 subtopics. Patients who have experienced out-of-hospital cardiac arrest have received help from the finder or the person, who called the emergency medical number. The work of a medical dispatcher is responsible and very stressful. Communication is a major link in the chain of cooperation between healthcare dispatchers and callers. Despite the obstacles encountered by callers or medical dispatchers, it turns out that necessary help was in most cases provided.
