Povzetek

Uvod: V diplomskem delu smo raziskovali princip mikrovalovne ablacije, kako jo izvajamo in kako celoten poseg poteka v Kliniki Golnik. Ob pregledu literature smo ugotovili, da je vloga radiološkega inženirja pri procesu skopo raziskana in nedefinirana, zato smo se odločili raziskati to področje. Zanimalo nas je predvsem, kakšna je vloga radiološkega inženirja pri posegu in kakšna predhodna znanja mora osvojiti. Namen: Namen diplomskega dela je na podlagi sistematičnega pregleda literature in opisa kliničnega primera predstaviti poseg mikrovalovne ablacije primarnega pljučnega tumorja. Metode dela: V diplomskem delu smo uporabili deskriptivno metodo z pregledom literature. Literaturo smo iskali v podatkovnih bazah, ki so bile dostopne na spletu in sicer Digitalna knjižnica Slovenije, Digitalna knjižnica Univerze v Ljubljani, Cobiss, Google Učenjak, Science Direct, PubMed, in Springer link. Znanje o mikrovalovni ablaciji in o vlogi radiološkega inženirja smo dopolnili z kvalitativno metodo in sicer z intervjujem z inženirji, zaposlenimi na oddelku Klinike Golnik. Rezultati: Za lažje razumevanje in prikaz postopka smo poseg opisali na kliničnem primeru pacienta, ki je zaradi pridruženih bolezni bil primeren kandidat le za zdravljenje z MWA. Zajeli smo anamnezo bolnika, potek ablacije, radiološke slike med postopkom kot tudi kontrolna spremljanja po posegu. Razprava in zaključek: Postopek ablacije, kot jo izvajajo v Kliniki Golnik, se bistveno ne razlikuje od postopkov, opisanih v literaturi. Glavni skupni točki sta priprava pacienta in izbira pacientov, primernih za poseg. Zapleti pri primeru posega so skladni z opisanim v literaturi, prav tako je primerljiv režim sledenja po ablaciji. Razlike smo opazili pri primerjavi tehničnih parametrov slikanja s CT in izbiro naprav za izvedbo posega. Vloga radiološkega inženirja je, da zagotovi kvalitetno in varno izvedbo postopka ablacije, zadosti tehničnim zahtevam, sodeluje pri pripravi pacienta in asistira radiologu pri posegu.

Sekundarni povzetek: Introduction: In this diploma thesis we explored the principles of CT guided microwave ablation. We described an example of how the procedure is performed at the Clinic Golnik. Reviewing the literature on the topic, we discovered that the role of the radiological technologist is poorly defined, so our aim was to research the technologists purpose and tasks during the procedure, as well as their knowledge requrements. Purpose: The purpose of this thesis is describing the process of microwave ablation of a primary pulmonary tumour, based on a systematic review of literature as well as presenting a clinical case. Methods: We combined a systematic review of literature and descriptive method. We searched the following databases: Digital library of Slovenia, Digital library of University of Ljubljana, Cobiss, Google Scholar, Science Direct, PubMed, and Springer link. We expanded our knowledge about the procedure and the radiographer's role with a qualitative method, a correspondance with the radiographers working at Clinic Golnik. Results: With the intention of a better understanding of the procedure, we presented a clinical case of a patient, that was due to their underlying medical condition, an appropriate candidate for MWA. We provided the patient's history as well as documented procedure steps. We included images created during the procedure, as well as follow up CT images. Discussion and conclusion: We came to a conclusion that the procedure protocol at the Clinic Golnik is very similar to the procedures described in the literature. The main similarities are the preparation of the patient, patient selection, complications after the procedure and follow up care and imaging. We found some differences comparing technical parameters, use of ablation devices. The radiographer's role is assuring quality and safety of the ablation procedure, responsibility for the technological aspect, preparation of the patient for the procedure as well as assisting the radiologist during the procedure.
