Povzetek
Uvod: Kenguru metoda je način nege nedonošenega otroka in predstavlja tesen, intimen stik med otrokom in mamo. S pomočjo zdravstvenih delavcev mati, lahko tudi oče, metodo izvaja na Enoti za intenzivno nego in terapijo novorojencev, kjer se srečuje s številnimi ergonomskimi izzivi. To so na primer neudobno oblikovan stol, okolje, ki ne zagotavlja ustreznega miru in zasebnosti, pomanjkljiva čustvena podpora zdravstvenih delavcev itn. Namen magistrskega dela je s pomočjo raziskave identificirati kognitivni, fizikalni in organizacijski vidik kenguru metode z vidika staršev.
Metode: V magistrski nalogi smo uporabili dve raziskovalni metodi. Sistematični pregled literature smo uporabili v teoretičnem delu, kjer smo s pomočjo podatkovnih baz podatkov Google učenjak, PubMed, Cinahl, ScienceDirect in Cobiss preučili obstoječo literaturo. Sledila je metoda anketiranja, pri kateri smo uporabili anonimni spletni vprašalnik 1KA. Vzorec je bil priložnostni in je vključeval 90 anketirancev. Raziskava je potekala od 13. 11. 2024 do 13. 12. 2024 , podatki pa so bili obdelani s programom SPSS.
Rezultati: Pred začetkom raziskave smo postavili pet hipotez. Pridobljene podatke anketnega vprašalnika smo analizirali. Ugotovili smo, da starši nedonošenih otrok niso zadovoljni z ureditvijo okolja za izvajanje kenguru metode, pri čemer so bili v povprečju nevtralnega mnenja glede osvetlitve, prezračevanja in toplote prostora, sedeži predstavljajo le delno udobje, prostor pa ne nudi zadostne zasebnosti. Starši so navezanost na otroka vzpostavili zelo različno – nekateri takoj po porodu, drugi pa nekaj mesecev kasneje, pri čemer dolžina izvajanja kenguru metode ni statistično povezana s hitrostjo vzpostavitve navezanosti. Wilcoxonovi testi so pokazali, da starši med kengurujčkanjem bistveno redkeje doživljajo negativne občutke (utrujenost, zaskrbljenost, razdražljivost itd.) kot ob sprejemu otroka na Enoto intenzivne nege in terapije. Raziskava je pokazala, da so starši nedonošenih otrok dobili dobro informacijsko in čustveno podporo s strani zdravstvenega osebja pri izvajanju kenguru metode. Zanimala nas je tudi uporaba telefona med izvajanjem kenguru metode. Ugotovili smo, da ga uporablja manj kot polovica, pri tem pa ga starši ne zaznavajo kot moteč dejavnik.
Razprava: Raziskava nam je podala uvid v ergonomski vidik trenutnega stanja s strani staršev nedonošenih otrok, ki na Enoti za intenzivno nego in terapijo izvajajo kenguru metodo. Raziskava izpostavlja možnosti za izboljšave fizikalnih dejavnikov ergonomije, ki se nanašajo na prostor, kjer so starši izvajali kenguru metodo. Smiselno bi bilo staršem zagotoviti mirno okolje, ki je brez motečih dejavnikov in nudi zasebnost, prav tako pa bi sedeži lahko bili oblikovani tako, da bi zagotavljali večje udobje in ustrezno držo telesa.

Ergonomic aspect of the kangaroo method care in preterm infants |

Introduction: Kangaroo care is a method of caring for a premature baby and represents a close, intimate contact between the baby and the mother. With the help of health workers, the mother, or father, can also perform the method in the Neonatal Intensive Care Unit, where she faces numerous ergonomic challenges. These include an uncomfortable chair, an environment that does not provide adequate peace and privacy, lack of emotional support from health workers, etc. The purpose of the master's thesis is to identify the cognitive, physical and organizational aspects of the kangaroo care method from the perspective of parents through research.
Methods: In the master's thesis, we used two research methods. A systematic literature review was used in the theoretical part, where we examined the existing literature using the databases Google Scholar, PubMed, Cinahl, ScienceDirect and Cobiss. This was followed by a survey method, in which we used an anonymous online questionnaire 1KA. The sample was random and included 90 respondents. The research was conducted from 13th November 2024 to 13th December 2024, and the data were processed using the SPSS program.
Results: Before starting the research, we made five hypotheses. We analyzed the data obtained from the survey questionnaire. We found that parents of premature babies are not satisfied with the arrangement of the environment for implementing the kangaroo care method, with an average of neutral opinions regarding the lighting, ventilation and heat of the room, the seats are only partially comfortable, and the room does not offer sufficient privacy. Parents established attachment to the child in very different ways - some immediately after birth, others a few months later, with the length of the kangaroo care method not statistically related to the speed of establishing attachment. Wilcoxon tests showed that parents experience negative feelings (fatigue, anxiety, irritability, etc.) significantly less often during kangaroo care than when the child is admitted to the Neonatal Intensive Care Unit. The study showed that parents of premature babies received good informational and emotional support from healthcare professionals when implementing kangaroo care. We were also interested in the use of the phone during kangaroo care. We found that less than half of the parents used it, and that the parents did not perceive it as a distracting factor.
Discussion: The research gave us an insight into the ergonomic aspect of the current situation on the part of parents of premature babies who use the kangaroo method at Neonatal Intensive Care Unit. The research highlights the possibilities for improvements in the physical factors of ergonomics, which relate to the space where the parents practiced the kangaroo method. It would make sense to provide parents with a quiet environment that is free of distractions and offers privacy, and the seats could also be designed to provide greater comfort and proper body posture. |
