Povzetek
Huntingtonova bolezen je neozdravljiva dedna motnja v delovanju možganov, ki prizadene gibanje, osebnost, razpoloženje, vedenje, čustva in kognicijo. V Sloveniji je trenutno okoli 120 pacientov s HB, ki imajo v napredovanem stadiju bolezni možnost nastanitve v štirih specializiranih domovih za oskrbo dementnih pacientov, vendar je čakalna doba dolga.
V diplomskem delu je predstavljena Huntingtonova bolezen in so opisane negovalne aktivnosti, ki jih izvajajo medicinske sestre v srednjem in napredovanem stadiju bolezni pri pacientih, ki živijo v domu za starejše občane. Prikazana je tudi vloga najožjih svojcev pri njihovi oskrbi.
V teoretičnem delu diplomskega dela je uporabljena deskriptivna metoda, v empiričnem delu pa kvalitativna metoda. V študiji primera pacientke s HB, ki je bila izvedena s pomočjo teoretičnega modela Virginie Henderson in njenih posameznih življenjskih aktivnosti, je bilo uporabljeno primarno (opazovanje z udeležbo) in sekundarno gradivo (razpoložljiva medicinska dokumentacija pacientke s HB). Raziskavo smo izvedli v domu starejših občanov.
Ugotovili smo, da je pacientka pri večini dnevnih aktivnosti popolnoma odvisna od tuje pomoči. Njeno izrazito nehotno gibanje (horejo) in njen nemir zdravijo z nevroleptiki, ki lahko še poslabšajo spoznavne motnje, razpoloženje in požiranje. Komunikacija med pacientko in negovalnim osebjem je zelo otežena. Najožji svojec pacientke dobro pozna bolezen in aktivno sodeluje pri njeni oskrbi. V času obiskov ji pomaga pri vseh aktivnostih, predvsem pa jo ves čas vzpodbuja.
Nega pacienta s HB v srednjem in napredovanem stadiju zahteva specialna znanja in multidisciplinarni pristop. Literatura, ki opisuje posebnosti pri negi teh pacientov je predvsem tuja, nekaj osnovnih informacij lahko dobijo negovalci v prispevkih, ki opisujejo nego dementnega bolnika in z ogledi videoposnetkov na spletnih straneh.

Theoretical starting-points: Huntington's disease is an incurable hereditary disorder manifesting itself in the functioning of the brain. It affects muscle coordination, personality, temper, behaviour, emotions and cognition. At the moment, there are approximately 120 such patients in Slovenia. For all those whose disease is in advanced condition, there is a possibility of going into one of the four homes which are specialized for treating patients suffering from dementia. But the waiting period is quite long.
Purpose: This diploma paper presents Huntington's disease and describes the nursing activities which are carried out by nurses who tend to the patients whose disease is in mid or advanced condition. These patients live in the homes for the aged. The diploma paper also deals with the role that the immediate relatives play in taking care of the patients.
Methodology of research: In the theoretical part of the diploma paper, the descriptive method is used; in the empirical part, the qualitative method is used. In the study of an example-patient suffering from HD, which was carried out by means of the theoretical model of Virginia Henderson and her individual life activities, the primary material (observing with participation) and the secondary material (available medical documentation of the patient suffering from HD ) were used. The research was carried out in a home for the aged.
Results: We came to the conclusion that the patient is totally dependent on the help from others in carrying out the majority of daily activities. Obvious involuntary movements (chorea) and restlessness are cured by neuroleptics, which can worsen the already existing recognition disorders, temper and swallowing. The communication between the patient and the nursing staff is very difficult. The nearest family member of the patient knows the disease well and takes an active part in the nursing process. During the visiting hours, he helps her in all activities, but most of all he offers constant encouragement.
Conclusion: Taking care of a patient suffering from HD in mid or advanced condition demands specialized knowledge and a multidisciplinary approach. The literature which describes all the specialities of nursing such a patient is mostly foreign. Some basic information can be obtained by the nursing staff from different articles and videos on the Internet. |
