Povzetek
Paliativna oskrba starostnika mora biti nepretrgana in celostna, zato ima medicinska sestra nepogrešljivo vlogo v paliativni zdravstveni negi. Zelo pomembno je pravočasno prepoznavanje motečih simptomov in le-te tudi pravočasno omejiti, da ne preidejo v nujna stanja. Paliativna nujna stanja lahko definiramo kot nujna stanja pri stanovalcih z neozdravljivo boleznijo v paliativnem stadiju bolezni. Z veliko verjetnostjo so povezana z napredovanjem osnovne bolezni, kar nam omogoča vnaprejšnje predvidevanje nastanka določenih nujnih stanj in pripravo na njih.
Namen diplomskega dela je predstaviti paliativno oskrbo v domu starejših ter opisati posebnosti urgentnega stanja pri starostniku v paliativni oskrbi.
Uporabili smo deskriptivno in kvantitativno metodo dela. Podatke smo zbrali s pomočjo anonimne ankete, katero so izpolnile medicinske sestre ter tehniki zdravstvene nege, zaposleni v domu starostnikov.
Rezultati raziskave so pokazali, da se medicinske sestre pogosto srečujejo s starostnikom v paliativni oskrbi. Pri starostniku se pojavljajo tako moteči simptomi kot tudi nujna stanja. Najpogosteje prepoznani moteči simptomi s strani anketiranih pri starostnikih so bolečina, oslabelost in težko dihanje. Zaradi pojava pljučne embolije, akutne dispneje in akutne krvavitve pa medicinske sestre najpogosteje kličejo službo nujne medicinske pomoči.
Paliativna oskrba starostnika mora temeljiti na dobrem sodelovanju z obolelim starostnikom, svojci in zaposlenim zdravstvenim kadrom. Dobro sodelovanje je osnova za lažje in pravilnejše odločitve, ki se skladajo z željami obolelega ob nastanku nujnega stanja.

Palliative care of the elderly should be a continuous and comprehensive, and therefore the nurse has indispensable role in palliative nursing care. It is very important the early identification of disruptive symptoms and these also limit in time that they don’t become emergencies. Palliative emergency situations can be defined as a necessary condition at residents with an incurable illness in the palliative stage of the disease. With high probability are associated with progression of the underlying disease, and therefore enabling us to anticipate in advance the occurrence of certain emergency situations and prepare for them.
The aim of the diploma work is to present palliative care in the nursing home and to describe the specifics of emergency situations of the elderly in palliative care.
We used a descriptive and quantitative method of work. Data were collected through an anonymous survey, which met the nurses and nursing technicians working in the nursing homes.
The results showed that nurses often encounter older people in palliative care. At the elderly appear so disturbing symptoms as well as emergencies. The most commonly identified disturbing symptoms by the respondents at the elderly are pain, weakness and shortness of breath. Due to the occurrence of pulmonary embolism, acute dyspnea and acute bleeding while nurses frequently call prehospital unit of emergency department.
Palliative care of the elderly should be based on good cooperation with the frail elderly, relatives and health personnel. Good cooperation is the basis for easier and more accurate decisions that comply with the wishes of the sick at the time of an emergency situation. |
