Povzetek
V Univerzitetnem kliničnem centru Ljubljana (UKCL) so l. 2011 začeli z uvajanjem nove informacijske tehnologije, imenovane PACS. Sistem je vezan na radiološko tehnologijo, digitalizacija diagnostičnega procesa omogoča optimizacijo procesa od napotitve bolnika na preiskavo do zaključenega izvida, slikovne priloge in arhiviranja celotnega postopka.
S postopnim uvajanjem sistema so delno prešli na »brezpapirni« sistem in z digitalizacijo tudi močno pohitrili postopke. Pojavile so se težave, ker vsi ostali podporni sistemi, kot so elektronska čakalna knjiga, obračun storitev in poročanje o čakalnih dobah, v tem projektu niso bili v celoti prilagojeni temu postopku.
S pomočjo sedemstopenjskega modela izboljševanja procesa smo optimizirali procese čakalne knjige, nadzora obračuna, skladiščenje in porabo sanitetnega materiala.
Želeli smo doseči pregledno strokovno, finančno poslovanje diagnostičnih in slikovnih enot v UKCL, da bi bilo skladno z navodili ZZZS in IVZ.
S pomočjo radiološkega informacijskega sistema, ki je bil osnova optimizacije urnikov za posamezne modalitete, smo raziskovali, kako le-te čim bolj izkoristili za boljše planiranje preiskav, preglednejšo realizacijo posegov in lažje načrtovanje posegov na različne modalitete. S tem smo želeli povečati storilnost in zmanjšati čakalne dobe, da bo proces naročanja optimiziran in stroški poslovanja nižji.
Zagotovili smo popoln obračun vseh storitev, kar je zelo pomembno za vse radiološke oddelke, saj se na to realizacijo veže odobritev programa, ki ga financira ZZZS za naslednje leto. Ravno tako smo nadgradili obračun storitev, da so vse radiološke storitve korektno obračunane. Čakalnih dob v tem trenutku nismo skrajšali, vendar smo se uspeli dogovoriti z zavarovalnico o povečanju programa za dejavnosti, ki imajo najdaljšo čakalno dobo.
Uvedba nove tehnologije vpliva na spremembo starih procesov in uvaja nove oblike dela, predvsem pa korenito spreminja miselnost zaposlenih in radiologijo kot diagnostične, slikovne in terapevtske metode.

In 2011, the University Medical Centre Ljubljana (UMCL) began introducing new information technology called PACS (Picture Archiving and Communication System). The system is associated with radiation technology, and the digitization allows the diagnostic process to be optimized; from the patient being referred to an investigation, to the completed test results, image attachment and archiving of the entire procedure.
The gradual introduction of the system provided a partial switch to a paperless system, and the digitization considerably expedited the procedures. The problems arose because all other support systems such as the electronic waiting list, charging for services and reporting on waiting times had not been fully adapted to this procedure.
With a 7-phase model for process improvement, we have optimized the waiting list processes, the accounts supervision processes, and the storage and use of medical supplies.
We have strived to achieve a more transparent, professional financial management of the diagnostic and imaging units in the UMCL to bring it into line with the instructions received from the ZZZS (Health Insurance Institute of Slovenia) and the IVZ (National Institute of Public Health).
Using the Radiology Information System, which constituted the basis for the optimization of the schedules for each imaging modality, it was examined as to how they should be used to improve the planning of investigations, to make intervention realization more transparent and to facilitate planning of interventions at each modality. This is aimed at increasing the performance and reducing waiting times in order to optimize the appointment process and decrease operational costs.
We have ensured complete accounting of all services, which is very important for all radiology departments, as the realization of this goal is related to the approval of the programme financed by the ZZZS for the following year. We have also upgraded the accounting of services, so that all radiology services are properly charged. We have not reduced the waiting times at this point; however, we have managed to reach an agreement with the Insurance Institute on an expansion of the programme for healthcare activities with the longest waiting times.
The implementation of the new technology has influenced the old processes and introduced new methods of work, and has in particular changed the attitude of employees and radiology itself as a diagnostic, imaging and therapeutic method. |
