Abstract
Uvod: Hipoksično ishemična encefalopatija (HIE) novorojenčkov je eden glavnih vzrokov umrljivosti, neonatalnih konvulzij in nevrorazvojnih motenj otrok. Zlati standard zdravljenja je še vedno terapevtska hipotermija (TH), eden od možnih načinov zdravljenja pa je tudi uporaba eritropoetina (EPO).
Metode: Sistematično smo pregledali znanstveno literaturo na izbrano temo. Uporabili smo deskriptivno metodo dela. Literaturo smo iskali v podatkovnih bazah PubMed, Medline, ScienceDirect, Cochrane Library, Wiley online library, Sage Journals in z iskalnikom Google Učenjak. Upoštevali smo smernice PRISMA. Raziskave smo razvrstili po nivojih z izbrano hierarhijo dokazov, jih kritično ocenili z uporabo orodja Joanna Briggs Institute ter jih prikazali opisno in v obliki evalvacijske tabele.
Rezultati: Skupno smo pridobili 64 zadetkov. V končno analizo smo vključili 13 raziskav. Ugotovili smo, da je učinek EPO pri novorojenčkih s HIE še vedno premalo raziskan in ne vpliva bistveno na zmanjšanje smrtnosti ali boljši nevrorazvojni izid ter da njegova uporaba ne more nadomestiti izvajanja TH.
Diskusija in zaključek: Za uporabo EPO pri novorojenčkih s HIE je še vedno premalo podatkov. Zato svetujemo raziskave v smeri, da bi lahko nadomestili izvajanje TH v okoljih z omejenimi viri ali pomembno pripomogli k boljšim rezultatom v kombinaciji s TH.

The effect of therapeutic hypothermia and erythropoietin on the neurodevelopmental outcome of neonates born with hypoxic ischemic encephalopathy |

Introduction: Neonatal HIE is one of the main causes of mortality, neonatal convulsions and neurodevelopmental disorders in children. The gold standard of treatment is still TH, and one of the potential treatment methods is the use of EPO.
Methods: We systematically reviewed the scientific literature on the selected topic. We used the descriptive method of work, the findings were compared with each other using the comparison method. Literature was searched in the databases PubMed, Medline, ScienceDirect, Cochrane Library, Wiley online library, Sage Journals and Google Scholar. We followed the PRISMA guidelines. Studies were classified the research by levels with a selected hierarchy of evidence, critically evaluated using the Joanna Briggs Institute tool and presented descriptively and in the form of an evaluation table.
Results: We obtained a total of 64 hits. We included 13 studies in the final analysis. We concluded that the effect of EPO in newborns with hypoxic ischemic encephalopathy is still insufficiently studied and that it does not significantly affect the reduction of mortality or better neurodevelopmental outcome and that its use cannot replace the implementation of TH.
Discussion and conclusion: There are still insufficient data for the use of erythropoietin in neonates with hypoxic ischemic encephalopathy. Therefore, we advise research in the direction that it could replace the implementation of TH in settings with limited resources or significantly contribute to better results in combination with TH. |
