Abstract

V diplomskem delu smo raziskali dinamične lastnosti sistemov za merjenje invazivnega arterijskega pritiska. V raziskavo smo vključili 16 bolnikov iz Klinike za infekcijske bolezni in vročinska stanja, enote intenzivne terapije Univerzitetnega Kliničnega Centra Ljubljana. Pri bolnikih smo šest zaporednih dni zajemali meritve dinamičnega odziva arterijskega sistema in skupno analizirali 85 meritev dinamičnih odzivov arterijske linije. Dinamika sistema je definirana kot odziv sistema na pravokotni impulz, tako imenovan »flush« test hitrega izpiranja sistema s fiziološko raztopino. Sistem se je odzival z različnimi koeficienti dušenja in resonančnimi frekvencami. Ugotavljamo, da so v 53 primerih meritve premalo dušene, v 19 primerih primerno dušene in v 13 primerih nesprejemljivo dušene. Pri bolnikih z uvedeno arterijsko linijo je pomembno, da medicinska sestra na enostaven način preizkuša koeficient dušenja in resonančno frekvenco sistema monitoringa s preizkusom hitrega izplakovanja, ki se izvaja ob bolnikovi postelji. Vrednosti koeficienta dušenja in resonančne frekvence je mogoče izračunati z izpisom krivulje na milimetrski papir, vendar za zagotavljanje pravilnega frekvenčnega odziva običajno zadošča že vizualni pogled.

Secondary abstract: The aim of this thesis was to explore dynamic characteristics of invasive arterial blood pressure measurement systems. Our research included 16 patients of the Clinic for Infectious Diseases and Febrile Illnesses of the Intensive Care Unit of the University Medical Centre Ljubljana. We took measurements of the dynamic response of the arterial system from our patients for six consecutive days, and in total analysed 85 measurements of the dynamic response of the arterial line. System dynamics are defined as responses of the system to the square waveform - fast flushing test of the system with saline. The system responded with different damping coefficients and resonant frequencies. Our conclusion is that 53 measurements showed underdamped dynamic responses, 19 measurements showed adequate dumping, whilst 13 measurements showed unacceptable dynamic responses. In case of patients with inserted arterial lines it is very important that nurses test the damping coefficient and the resonant frequency of the monitoring system with a simple method of fast flushing performed at the patient's bedside. Values of the damping coefficient and the resonant frequency can be computed by printing the wave on graph paper. However, to assure the correct frequency response, a visual inspection is generally adequate.
